摘 要:缝牵引成骨术(SDO)是一种充分利用骨缝生长潜力的手术技术,可减少手术创伤,但常因骨缝内新生骨量不足导致治疗周期延长、术后复发率升高等。骨形态发生蛋白-2(BMP-2)是促进骨髓间充质干细胞向成骨细胞分化的关键因子,SDO内源性BMP-2的表达可促进牵张骨缝内新骨生成,重组人BMP-2(rhBMP-2)的局部应用不仅需要构建合适的载体,还极易导致骨缝融合的发生。深入探究SDO内源性BMP-2的表达规律及其影响因素,确定rhBMP-2对牵张骨缝的成骨作用规律,有助于早日实现BMP-2相关治疗措施在SDO中的临床应用,为缩短疗程、降低复发率提供新的方法和思路。Sutural distraction osteogenesis(SDO)is a surgical technique that makes full use of the growth potential of bone sutures.While reducing surgical trauma,it often has some problems such as long treatment time and high recurrence rate due to the insufficient new bone mass in the sutures.Bone morphogenetic protein-2(BMP-2)is a key factor in promoting the differentiation of bone marrow mesenchymal stem cells into osteoblasts.During SDO,the expression of endogenous BMP-2 can promote the formation of new bone in the sutures.Local application of recombinant human BMP-2(rhBMP-2)not only requires the suitable carrier,but also easily causes premature sutural fusion.In-depth exploration of the expression pattern of endogenous BMP-2 and its influencing factors in SDO,and the law of osteogenesis of rhBMP-2 on sutures will help to realize the clinical application of BMP-2-related treatment measures in SDO as soon as possible,and provide new methods and ideas for shortening the treatment time and reducing the recurrence rate.
