β-连环蛋白、c-myc、p16在鼻腔鼻窦黏膜黑色素瘤中的表达及临床意义

摘　　要：目的探讨β-连环蛋白(β-catenin)、c-myc、p16在鼻腔鼻窦黏膜黑色素瘤(sinonasal mucosal melanoma,SNM)中的表达及临床意义。方法采用免疫组化Multimer法检测85例未接受放化疗和靶向治疗的原发性SNM组织中β-catenin、c-myc和p16的表达,分析其与临床病理特征、预后的关系,并分析三者表达的相关性。结果SNM中β-catenin、c-myc、p16表达阳性患者的肿瘤淋巴结转移率分别为69.7%、74.1%和76.4%,均高于阴性表达患者,差异有统计学意义(P均<0.05)。c-myc阳性、p16强阳性表达患者的总生存期和无病生存期低于阴性表达患者(P均<0.05),β-catenin与生存期无明显相关性。c-myc的表达与β-catenin的表达呈正相关(r=0.234,P=0.027)。结论SNM中β-catenin、c-myc、p16阳性表达是肿瘤淋巴结转移的重要预测因子。c-myc阳性、p16强阳性表达患者总生存期和无病生存期较短,提示预后差,可以作为判断SNM预后的重要参考指标。Objective To investigate the expression ofβ-catenin,c-myc and p16 in sinonasal mucosal melanoma(SNM)and its clinical significance.Methods Immunohistochemical Multimer was used to detect the expression ofβ-catenin,c-myc and p16 in 85 primary SNM samples that had not received chemoradiotherapy or targeted therapy.The correlation between the expression ofβ-catenin,c-myc,p16 and clinicopathological features and prognosis was analyzed.The correlation between the expression of β-catenin,c-myc and p16 was analyzed.Results The lymph node metastasis rates ofβ-catenin,c-myc and p16 positive patients in SNM were 69.7%,74.1%and 76.4%,respectively,which were all higher than those of patients with low expression,and the differences were statistically significant(P all<0.05).The overall survival and disease-free survival of c-myc positive patients and p16 strongly positive patients were lower than those of negative patients(P all<0.05).There was no significant correlation between expression ofβ-catenin with survival rate.The expression of c-myc was positively correlated with the expression ofβ-catenin(r=0.234,P=0.027).Conclusion The positive expression ofβ-catenin,c-myc and p16 in SNM was an important predictor of lymph node metastasis.Shorter overall survival and disease-free survival in patients with positive c-myc expression and strongly positive p16 expression suggest poor prognosis,which can be used as an important reference index to judge the prognosis of SNM.
