复方阿替卡因在儿童牙体牙髓无痛治疗中的应用效果分析

摘　　要：目的分析在儿童牙体牙髓病的无痛治疗中应用复方阿替卡因的效果。方法用计算机产生随机数分组法将方便选取的该院2019年4月—2021年2月间收治的86例儿童牙体牙髓病患儿平均分成两组进行麻醉研究。比对组43例患儿,采用利多卡因治疗,探究组43例患儿,给予复方阿替卡因治疗。比较两组麻醉效果、心率、平均动脉压指标、麻醉安全性及疼痛评分。结果探究组麻醉总有效率(95.35%)高于比对组(74.42%),差异有统计学意义(χ^(2)=4.073,P=0.043);探究组T1、T2时刻HR(88.22±6.94)次/min、(77.46±5.14)次/min、MAP(90.92±6.24)mmHg、(69.24±6.75)mmHg均低于比对组,而T3时刻比对组HR水平(74.54±6.43)次/min显著降低,MAP(67.57±4.18)mmHg与标准水平相比明显更低,差异有统计学意义(t=2.872、22.833、2.774、22.063、6.678、8.382,P<0.05);且探究组用药后不良反应发生率(4.65%)明显低于比对组,差异有统计学意义(χ^(2)=4.073,P=0.043);探究组疼痛程度明显轻于比对组,差异有统计学意义(P<0.05)。结论在儿童牙体牙髓病的治疗中采用复方阿替卡因能取得较好的麻醉效果,对心率及平均动脉压的影响较小,且用药后安全性良好,值得推广应用。Objective To analyze the effect of compound articaine in the painless treatment of children's dental pulp disease.Methods Used computer-generated random number grouping method,conveniently selection 86 children with dental and endodontic diseases admitted to the hospital from April 2019 to February 2021 were equally divided into two groups for anesthesia research.A total of 43 children in the comparison group were treated with lidocaine,and a total of 43 children in the exploration group were treated with compound articaine.The anesthesia effect,heart rate and mean arterial pressure indexes,anesthesia safety and pain score were compared between the two groups.Results The total effective rate of anesthesia in the exploration group(95.35%)was higher than that in the comparison group(74.42%),the difference was statistically significant(χ^(2)=4.073,P=0.043).The HR(88.22±6.94)times/min,(77.46±5.14)times/min,MAP(90.92±6.24)mmHg and(69.24±6.75)mmHg of the exploration group at T1 and T2 were all lower than those of the control group,at T3,the HR level of the comparison group(74.54±6.43)times/min was significantly lower,and the MAP(67.57±4.18)mmHg was significantly lower than the standard level,the difference was statistically significant(t=2.872,22.833,2.774,22.063,6.678,8.382,P<0.05).And the incidence of adverse reactions in the exploration group(4.65%)was significantly lower than that in the comparison group,the difference was statistically significant(χ^(2)=4.073,P=0.043).The pain degree in the exploration group was significantly lighter than that in the comparison group,the difference was statistically significant(P<0.05).Conclusion Compound articaine can achieve good anesthesia effect in the treatment of children's dental pulp disease,has little effect on heart rate and mean arterial pressure,and has good safety after medication,which is worthy of popularization and application.
