T1期非肌层浸润性膀胱癌组织中FGFR3与CDKN2A的表达对预后的预测价值

摘　　要：目的 观察成纤维细胞生长因子受体3(FGFR3)和细胞周期蛋白依赖性激酶抑制剂2A(CDKN2A)在T1期非肌层浸润性膀胱癌(NMIBC)的表达情况,及其对预后的预测价值。方法 回顾性收集62例T1期NMIBC患者的肿瘤组织蜡块,所有患者均行经尿道膀胱电切术。应用实时荧光定量聚合酶链反应(RT-qPCR)检测FGFR3和CDKN2A的mRNA表达,并分析与临床资料的关系。分别应用角蛋白5(KRT5)和角蛋白20(KRT20)作为基底亚型和管腔亚型的标志物。结果 FGFR3与CDKN2A mRNA的表达无相关,与KRT5(rs=0.297,P=0.005)和KRT20mRNA的表达均呈正相关(rs=0.314,P<0.001);FGFR3高表达与肿瘤分级呈负相关(rs=-0.37,P<0.001);CDKN2A与KRT5mRNA的表达呈负相关(rs=-0.331,P=0.021)。FGFR3mRNA高表达组的患者具有更差的肿瘤无复发(RFS),但肿瘤无进展生存(PFS)和总生存(OS)有所提高,CDKN2A mRNA高表达组的患者具有更差的OS。高龄(≥70岁)和FGFR3高表达与复发风险增加显著相关;FGFR3高表达与降低OS显著相关。结论 对于T1期NMIBC患者,FGFR3mRNA高表达可能预示着较差的RFS,但OS更好。Objective To observe the expression of Fibroblast growth factor receptor 3(FGFR3)and cyclin dependent kinase inhibitor 2A(CDKN2A)in T1stage non-muscle invasive bladder cancer(NMIBC)and its prognostic value.Methods The formalin fixation and paraffin embedding of 62patients with T1stage NMIBC were collected retrospectively.All patients underwent transurethral resection of bladder.The mRNA expressions of FGFR3and CDKN2Awere detected by real time quantitative polymerase chain reaction(RT-qPCR),and the relationship with clinical data was analyzed.Keratin 5(KRT5)and keratin 20(KRT20)were used as markers of basal subtype and lumen subtype,respectively.Results FGFR3was not correlated with the expression of CDKN2A mRNA,but positively correlated with the expression of KRT5(rs=0.297,P=0.005)and KRT20mRNA(rs=0.314,P<0.001);The high expression of FGFR3was negatively correlated with tumor grade(rs=-0.37,P<0.001);CDKN2A was negatively correlated with the expression of KRT5mRNA(rs=-0.331,P=0.021).Patients with high FGFR3mRNA expression group had worse RFS,but PFS and OS increased,and patients with high CDKN2A mRNA expression group had worse OS.Advanced age(≥70years)and high expression of FGFR3were significantly associated with increased risk of recurrence;The high expression of FGFR3 was significantly correlated with the decrease of OS.Conclusion For patients with T1stage NMIBC,high expression of FGFR3mRNA may predict poor RFS,but OS is better.
