LncRNA H19和VEGF-b在糖尿病视网膜病变患者中的表达及临床意义

摘　　要：目的 探究糖尿病视网膜病变(DR)患者中LncRNA H19和VEGF-b表达水平以及临床意义。方法 选取2型糖尿病患者120例分为糖尿病无视网膜病变组(DM组)、非增殖型糖尿病视网膜病变组(NPDR组)和增殖型糖尿病视网膜病变组(PDR组),每组各40例患者;选取特发性黄斑裂孔且不伴糖尿病的患者40例为对照组(NC组)。分别采用ELISA法和RT-qPCR法检测各玻璃体和血清样品中VEGF-b和LncRNA H19的表达水平;免疫组化检测LncRNA H19和VEGF-b阳性表达;通过Logistic多元回归分析VEGF-b和LncRNA H19是否为DR的危险因素。结果 免疫组化结果表明NC组患者视网膜组织中LncRNA H19阳性表达数高于PDR组(P<0.05),VEGFb阳性表达数低于PDR组(P<0.05)。ELISA检测结果显示NC、DM、NPDR、PDR组患者玻璃体和血清中VEGF-b表达水平依次升高,差异有统计学意义(P<0.05),而LncRNA H19相对表达水依次降低,差异有统计学意义(P<0.05)。Western blo结果显示PDR组患者膜组织中VEGF-b蛋白表达水平显著高于NC组(P<0.05)。相关性分析结果提示血清中VEGF-b浓度与血清中IL-6水平呈线性正相关(r=0.899,P<0.01),而LncRNA H19与IL-6呈负相关(r=-0.858,P<0.01)。结论 DR患者玻璃体和血清中及PDR患者视网膜组织中VEGF-b表达水平升高,而LncRNA H19相对表达水平下降,且为DR的危险因素,其可能参与了DR的发生发展过程。Objective To investigate the expression of LncRNA H19and VEGF-b in patients with diabetic retinopathy(DR)and their clinical significance.Methods 120patients with type 2diabetes mellitus were divided into diabetic retinopathy group(DM group),non value-added diabetic retinopathy group(NPDR group)and proliferative diabetic retinopathy group(PDR group),40patients with idiopathic macular hole without diabetes mellitus were selected as control group(NC group).The expression levels of VEGF-b and LncRNA H19in vitreous and serum samples were detected by ELISA kit and RT-qPCR;the positive expressions of LncRNA H19and VEGF-b were detected by immunohistochemistry;the risk factors of Dr were analyzed by logistic multiple regression.Results Immunohistochemical results showed that the positive expression of LncRNA H19in the retina of NC group was higher than that of PDR group(P<0.05),and the positive expression of VEGF-b was lower than that of PDR group(P<0.05).The results of ELISA showed that the expression of VEGF-b in vitreous and serum of NC,DM,NPDR and PDR groups increased in turn,the difference was statistically significant(P<0.05),while the relative expression of LncRNA H19decreased in turn,the difference was statistically significant(P<0.05).Western blot results showed that the expression level of VEGF-b protein in PDR group was significantly higher than that in NC group(P<0.05).Correlation analysis showed that serum VEGF-b concentration was positively correlated with serum IL-6level(r=0.899,P<0.01),while LncRNA H19was negatively correlated with IL-6level(r=-0.858,P<0.01).Conclusion The expression level of VEGF-b in vitreous and serum of Dr patients and membrane tissue of PDR patients is increased,while the relative expression level of LncRNA H19is decreased,which is a risk factor of DR and may be involved in the occurrence and development of Dr.
