摘　　要：目的 探究脑源性神经营养因子(brain-derived,neurotrophic factor,BDNF)、微小核糖核酸-381-3p(miR-381-3p)、血栓素A2(Thromboxane A2,TXA2)在抑郁症中的表达及在乌灵胶囊干预效果评估中的价值。方法 2020年3月-2021年3月选取50例在秦皇岛市第一医院就诊的抑郁症患者为抑郁组,同时期在我院体检的50例健康人为健康组,抑郁症组又根据治疗方式的不同分为27例乌灵胶囊治疗组、23例对照治疗组,乌灵胶囊治疗组给予患者口服乌灵胶囊联合草酸艾司西酞普兰,对照治疗组使用草酸艾司西酞普兰。实时荧光定量PCR检测miR-381-3p、BDNF相对表达量,放射免疫分析法检测TXA2表达。观察BDNF、miR-381-3p、TXA2在两组中的表达,同时统计乌灵胶囊对抑郁症干预效果的相关性。受试者特征曲线(receiver operator characteristics curve,ROC)分析BDNF、miR-381-3p、TXA2对乌灵胶囊效果的预测价值。结果 健康组BDNF、miR-381-3p高于抑郁组,TXA2低于抑郁组(P<0.05)。与对照组相比,联合组BDNF、miR-381-3p较高,TXA2较低(P<0.05)。在5-羟吲哚乙酸(5-hydroxyindoleacetic acid,5-HIAA)≥0.22,去甲肾上腺素(Nor-epinephrine,NE)≥50,多巴胺(Dopamine,DA)≥424,抑郁自评量表<53时BDNF、miR-381-3p升高,TXA2降低;5-HIAA<0.22,NE<50,DA<424,抑郁自评量表≥53时BDNF、miR-381-3p降低,TXA2升高(P<0.05)。与BDNF、miR-381-3p、TXA2单一预测相比,联合预测对抑郁症使用乌灵胶囊效果诊断效能较高(P<0.05)。结论 抑郁症患者使用乌灵胶囊干预后,BDNF、miR-381-3p、TXA2表达异常,有效患者BDNF、miR-381-3p较高,TXA2较低,无效则相反,三者与乌灵胶囊效果的治疗效果具有一定的相关性,通过BDNF、miR-381-3p、TXA2联合检测对于抑郁症有一定的诊断价值。Objective To explore the expression of brain-derived neurotrophic factor(BDNF),minute ribonucleic acid-381-3p(miR-381-3p),Thromboxane A2(TXA2)in depression and in the evaluation of intervention efficacy of Uling capsules.Methods From March 2020to March 2021selected 50depression patients in our hospital for depression group,the same period in our hospital 50cases of health people for health group,depression group is divided into urin capsule treatment group in 27cases,23control treatment group,urling capsule treatment group gave patients using ulin capsule combined with oxcitalopram,control treatment group using oxcitalopram.Relative expression of miR-381-3p and BDNF was determined by quantitative real-time PCR,and TXA2expression was detected by radioimmunoassay.BDNF,miR-38Expression of 1-3p and TXA2in both groups and correlation with the effect of Uling capsule on the depression intervention.The predictive value of subject characteristic curves(receiver operator characteristics curve,ROC)analysis on the effect of BDNF,miR-381-3p,and TXA2on Uling capsules.Results In the healthy group,BDNF and miR-381-3p were higher than in the depressed group,and TXA2was lower than in the depressed group(P<0.05).In the combination group,higher BDNF,miR-381-3p and lower TXA2were observed when compared to controls(P<0.05).At 5-hydroindindacetic acid(5-hydroxyindoleacetic acid,5-HIAA)0.22,norepinephrine(Nor-epinephrine,NE)50,dopamine(Dopamine,DA)424,BDNF at depression self-evaluation scale<53,Higher miR-381-3p,TXA2;5-HIAA<0.22,NE <50,DA <424,BDNF,miR-381-3p,and TXA2increased in the depression self-evaluation scale 53(P<0.05).Compared with the single prediction of 38TXA2,BDNF,miR-381-3p and TXA2(P<0.05).Conclusion BDNF,miR-381-3p,and TXA2 were abnormal after intervention in depressed patients,BDNF,miR-381-3p were high,TXA2was low,and ineffective.The three have some correlation with the therapeutic effect of Wuling capsule effect,and BDNF,miR-381-3p and TXA2had some diagnostic value for depression.
