摘　　要：目的探讨鼻腔鼻窦乳头状瘤-嗜酸细胞型(sinonasal papillomas-oncocytic type,SPO)的临床病理学特征、诊断、鉴别诊断、治疗及预后。方法收集15例SPO临床资料,采用HE及免疫组化EnVision两步法进行检测,分析其临床病理特征并复习相关文献。结果15例SPO患者年龄43~71岁,平均62.5岁,男女比约为3∶1。SPO具有典型的组织学特点,肿瘤细胞上皮通常有2~8层,胞质丰富,嗜酸性、颗粒状。表面外层细胞可见纤毛,特征性改变是上皮层内含有微脓肿和(或)含有黏液的小囊。免疫表型:肿瘤细胞CD44、AMA、GLUT1均(),Ki-67增殖指数2%~5%。结论SPO是鼻腔鼻窦乳头状瘤的少见类型,具有独特的组织学特征,有复发和恶变潜能,治疗以手术切除为主。Purpose To explore the pathologic features,histogenesis,diagnosis and differential diagnosis,therapy and prognosis of the sinonasal papillomas-oncocytic type(SPO)in single medical centre.Methods The clinical data of 15 cases of SPO were collected and detected by HE and immunohistochemical EnVision two-step method.The clinicopathological features were analyzed and the relevant literatures were reviewed.Results The age of 15 patients with SPO ranged from 43 to 71 years,with an average age of 62.5 years.The ratio of male to female was about 3∶1.Histologically,they had typical morphological characteristics of SPO.The epithelium of tumor cells usually had 2 to 8 layers with abundant cytoplasm and was eosinophilic and granular.Cilia were seen on the outer cells of the surface.A more characteristic features was small cysts containing micro abscesses and/or mucus within the epithelial layer.By immunochemistry stain,the tumor cells expressed CD44,AMA,GLUT1 and the proliferating index of Ki-67 was 2%-5%.Conclusion SPO is an uncommon type of sinonasal papilloma with distinctive histomorphologic features and potential for recurrence and malignant transformation,and current treatment is based on surgical excision as the first choice.
