角膜缘干细胞与带蒂结膜瓣移植术治疗翼状胬肉的疗效比较

摘　　要：目的探讨角膜缘干细胞与带蒂结膜瓣移植术治疗翼状胬肉的临床效果。方法选择2018年1月-2019年1月我院收治的翼状胬肉患者70例,采用随机数字表法分为两组,各35例。两组均行胬肉切除术,对照组实施带蒂结膜瓣移植术,观察组实施角膜缘干细胞移植术,比较两组角膜修复时间、复发率及并发症。结果观察组角膜修复时间(<14d、14-21d、22-30d患者占比分别为40.00%、57.14%、2.86%)短于对照组(<14d、14-21d、22-30d患者占比分别为0.00%、42.86%、57.14%),差异有统计学意义(Z=5.606,P=0.000);观察组术后4个月内复发率(2.86%)低于对照组(22.86%),差异有统计学意义(χ^(2)=4.590,P=0.012);观察组并发症发生率(5.71%)低于对照组(25.71%),差异有统计学意义(χ^(2)=5.285,P=0.022)。结论角膜缘干细胞移植术治疗翼状胬肉效果较好,可促进患者角膜修复,减少术后并发症,降低复发率。Objetive To investigate the clinical effect of limbal stem cells and pedicled conjunctiva flap transplantation in the treatment of pterygium.Methods 70 patients with pterygium admitted to our hospital from January 2018 to January 2019 were selected.Pterygium resection was performed in both groups,pedicled conjunctival flap transplantation was performed in the control group,and limbal corneal stem cell transplantation was performed in the observation group,and corneal repair time,recurrence rate and complications were compared between the two groups.Results The corneal repair time(40.00%,57.14%and 2.86%of patients with<14d,14-21d and 22-30d,respectively)of the observation group was shorter than that of the control group(0.00%,42.86%and 57.14%of patients with<14d,14-21d and 22-30d,respectively),and the difference was statistically significant(Z=5.606,P=0.000).The recurrence rate of the observation group(2.86%)was lower than that of the control group(22.86%)within 4 months after the operation,and the difference was statistically significant(χ^(2)=4.590,P=0.012).The incidence of complications in the observation group(5.71%)was lower than that in the control group(25.71%),and the difference was statistically significant(χ^(2)=5.285,P=0.022).Conclusion Limbal stem cell transplantation was effective in the treatment of pterygium.
