静止期翼状胬肉两种手术方式的比较

摘　　要：目的比较静止期翼状胬肉选择单纯切除术与自体角膜缘干细胞移植术两种手术方式治疗效果。方法A组:静止期翼状胬肉患者28例28眼行翼状胬肉单纯切除手术,B组:静止期翼状胬肉患者32例32眼行翼状胬肉切除伴自体角膜缘干细胞移植术。两组分别对手术整体时间、术后角膜创面上皮愈合时间、术后复发率进行比较。结果A组手术时间(21.54±3.636)min明显短于B组(33.06±4.697)min,差异有统计学意义(t=10.515,P<0.05);两组角膜创面上皮愈合所需时间比较,A组(6.46±1.07)d,B组(6.13±1.137)d,差异无统计学意义(t=1.139,P>0.05),术后1个月,3个月翼状胬肉复发率比较,差异无统计学意义(χ^(2)=2.365,2.435,P均>0.05)。结论对静止期翼状胬肉手术方式选择上,单纯切除术手术时间短,术后复发率与自体角膜缘干细胞移植术无明显差异。在临床上,对于高龄的静止期翼状胬肉患者,不能耐受长时间手术的,选择单纯切除手术方式也是不错的选择。Objective Compare simple excision with autograft limbal stem cell transplantation for the operation effects of static pterygium.Methods Group A:28 patients(28eyes)static pterygium which excise alone,group B:32 patients(32eyes)static pterygium which excise with autologous limbal stem cell transplantation.The whole operation time,postoperative corneal wound epithelial healing time and postoperative recurrence rate were compared between the two groups.Results The operation time of group A(21.54±3.636)min was significantly shorter than that of group B(33.06±4.697)min,(t=10.515,P<0.05);The corneal epithelium healing time of group A was(6.46±1.07)d,and group B was(6.13±1.137)d,There was no significant difference in corneal epithelium healing time and postoperative recurrence rate between the two groups(t=1.139,P>0.05;χ^(2)=2.365,2.435,all P>0.05).Conclusion The time of simple pterygium resection was short,and there was no significant difference of postoperative recurrence rate between simple pterygium resection and autologous limbal stem cell transplantation.In clinic,simple excision was also a good choice for elderly patients with static pterygium who cannot tolerate long operation time.
