摘 要:目的:探究给予早期脑卒中患者基于左右制衡理论的镜像疗法对其体感诱发电位及上肢功能重建应用价值。方法:选取我科98例早期脑卒中后上肢功能障碍患者随机分组。对照组49例给予常规强制性运动疗法训练,观察组49例给予基于左右制衡理论的镜像疗法,对比两组患者上肢功能:Bruunstrom分期、上肢简化Fugl-Meyer量表、Carroll手功能评定(Upper extremity function test,UEFT);上肢协调功能:肘屈曲协同收缩率、肘伸展协同收缩率;体感诱发电位:N20潜伏期、波幅。结果:干预后观察组手Bruunstrom分期、上肢Bruunstrom分期、肘屈曲协同收缩率、肘伸展协同收缩率、Fugl-Meyer、UEFT量表评分、N20波幅高于对照组,N20潜伏期低于对照组(P<0.05)。结论:早期脑卒中以制衡理论为基础进行镜像干预可有效改善大脑诱导电位,提高上肢协调运动能力。Objective:To explore the application value of mirror therapy based on the theory of balance between left and right in somatosensory evoked potential and upper limb function reconstruction of early stroke patients.Methods:98 patients with upper limb dysfunction after early stroke in our department were randomly divided in two groups.49 patients in the control group were given routine mandatory exercise therapy training,and 49 patients in the observation group were given mirror therapy based on left and right balance theory.The upper limb functions:Bruunstrom staging,Upper Extremity Function Test(UEFT),Simplified FuGL-Meyer Scale;Coordination function of upper limbs:cooperative contraction rate of elbow flexion,cooperative contraction rate of elbow extension;Somatosensory evoked potential:N20 latency,amplitude were compared between the were two groups.Results:After intervention,the observation group had higher hand Bruunstrom stage,upper limb Bruunstrom stage,elbow flexion cooperative contraction rate,elbow extension cooperative contraction rate,FUGL-Meyer,UEFT scale score and N20 amplitude compared with the control group,while the N20 latency was lower than the control group(P<0.05).Conclusion:Mirror image intervention based on the theory of balance and balance can effectively improve the brain induced potential and the coordination of upper limb motor ability in early stroke.
