微钛钉种植体支抗压低前牙治疗露龈笑者的中期随访

摘　　要：目的探究微钛钉种植体支抗压低前牙对露龈笑者的矫正效果。方法选取2018年7月至2019年6月郑州市第七人民医院75例露龈笑者,根据随机数字表法分为两组,对照组(37例)予以口外支抗治疗,研究组(38例)予以微钛钉种植体支抗进行压低前牙方式治疗,比较两组矫治前及矫治1年后矫治前后微笑露龈高度差值、上前牙(尖牙、中切牙、侧切牙)的牙冠长度、红色—白色美学标准(PES-WES)评分。结果研究组矫治前后微笑露龈高度差值为(2.75±0.52)mm,对照组矫治前后微笑露龈高度差值(1.98±0.45)mm,两组矫治前后微笑露龈高度差值比较,差异有统计学意义(P<0.001);两组矫正前上前牙的尖牙、侧切牙及中切牙牙冠长度比较,差异无统计学意义(P>0.05);矫治1年后研究组上前牙的尖牙、侧切牙、中切牙牙冠长度均短于对照组,差异有统计学意义(P<0.05);两组矫正前PES-WES评分比较,差异无统计学意义(P>0.05);矫治1年后研究组PES-WES评分高于对照组,差异有统计学意义(P<0.05)。结论微钛钉种植体支抗压低前牙应用于露龈笑者,可缩短上前牙牙冠长度,有效改善患者微笑露龈问题,提高美学效果,在临床治疗中具有重要意义。Objective To analyze the application effect of micro-titanium nail implant anchorage on anterior teeth intrusion in patients with gingival smile.Methods A total of 75 patients with gummy smile who were treated in Zhengzhou Seventh People‘s Hospital from July 2018 to June 2019 were divided into two groups according to the random number table,the control group(37 cases)was treated with extraoral anchorage,and the study group(38 cases)was treated with micro-titanium nail implant anchorage on anterior teeth intrusion.The crown length of the upper anterior teeth(canine,central incisor and lateral incisor)before and after 1 year of treatment,the difference of smile gum height before and after treatment,and the red-white aesthetic standard(PES-WES)score were compared between the two groups.Results The difference in smile gingival height before and after correction was(2.75±0.52)mm in the study group and(1.98±0.45)mm in the control group,and the difference in smile gingival height before and after correction was statistically significant(P<0.001);the difference in the length of the cusps,lateral incisors and crowns of the central incisors before correction was not statistically significant between the two groups(P>0.05).one year after treatment,the crown lengths of the cuspids,lateral incisors and central incisors in the study group were shorter than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the PES-WES scores between the two groups before orthodontic treatment(P>0.05);after 1 year of orthodontic treatment,the PES-WES scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of micro-titanium nail implant anchorage on anterior teeth intrusion in patients with gingival smile can shorten the crown length of the upper anterior teeth,effectively improve the patients’s gingival smile and improve the aesthetic effect,which is of grea
