萌出诱导矫治器对替牙期牙列拥挤髁状突位置、PAR指数的影响

摘　　要：目的探讨萌出诱导矫治器对替牙期牙列拥挤髁状突位置、正畸标准指数(PAR)的影响。方法选取黄河水利委员会黄河中心医院口腔科自2018年1月至2018年12月收治的100例替牙期牙列拥挤患儿,按照随机数表达法将其分为研究组(50例)与对照组(50例),对照组采用固定矫治器,并以Hawley氏保持,研究组接受萌出诱导矫治器矫治,观察两组治疗前后三维牙颌数字模型,比较髁状突位置、PAR指数变化。结果研究组、对照组平均疗程为(6.78±2.61)个月、(14.19±2.57)个月,比较差异有统计学意义(P<0.05);治疗后,两组前、后牙列拥挤度显著改善,差异有统计学意义(P<0.05),研究组改善幅度显著大于对照组,差异有统计学意义(P<0.05);治疗后,两组关节间隙、PAR值等明显改善,研究组改善幅度明显大于对照组,差异有统计学意义(P<0.05)。结论萌出诱导矫治器可以显著改善替牙期牙列拥挤,能够有效改善髁状突位置,建立正常覆牙合、覆盖、牙齿、颜面正常发育、改善牙列拥挤等,效果确切,值得在临床应用。Objective To investigate the effect of eruption induction appliance on the position of crowded condyle and PAR(orthodontic standard index)during dentition replacement.Methods A total of 100 children with dentition crowding treated in the Department of Stomatology of the Yellow River Central Hospital of the Yellow River Water Conservancy Commission from January to December 2018 were randomLy divided into study group(N=50)and control group(N=50).The control group was treated with fixed appliance and Hawley’s appliance,while the study group was treated with eruption induction appliance.Three-dimensional dental digital models of the two groups were observed before and after treatment,and the changes of condylar position and PAR index were compared.Results The average course of treatment in the study group and the control group was(6.78±2.61)months and(14.19±2.57)months,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the crowding degree of anterior and posterior dentition between the study group and the control group(P>0.05).After treatment,the crowding degree of anterior and posterior dentition of the two groups was significantly improved,and the difference was statistically significant(P<0.05).The improvement in the study group was significantly higher than that in the control group(P<0.05).Before treatment,there was no significant difference in joint space and PAR between the study group and the control group(P>0.05).After treatment,the joint space and PAR of the two groups were significantly improved,and the improvement extent of the study group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Eruption induction appliance can significantly improve dentition crowding during dentition replacement,effectively improve condylar position,establish normal overbite,cover,normal tooth and facial development,improve dentition crowding,etc.The effect is accurate and worthy of clinica
