摘  要:目的探讨上皮钙黏素(E-cadherin)、微管不稳定蛋白1(STMN1)和H^(+)/K^(+)-ATP酶β亚基(ATP4B)在胃癌细胞中表达的关系及其临床意义。方法免疫组化检测51例胃癌及对应癌旁组织中E-cadherin、AKT1、FOXO3a、STMN1和ATP4B的表达,分析其相关性及与临床病理特征的关系;通过受试者工作特征(receiver operating characteristic,ROC)曲线评估E-cadherin、STMN1和ATP4B联合诊断价值;构建E-cadherin过表达的胃癌AGS细胞稳转株,Western blot法检测两组细胞中E-cadherin、FOXO3a、AKT1、p-AKT、STMN1、和ATP4B蛋白表达;CCK-8法检测5-FU对两组细胞活力的影响。结果与癌旁组织相比,胃癌组织中E-cadherin、FOXO3a和ATP4B的阳性表达率降低(P<0.05),AKT1、STMN1的阳性表达率升高(P<0.05)。E-cadherin与FOXO3a、ATP4B的表达呈正相关(r=0.394,P=0.004;r=0.324,P=0.020),ATP4b与FOXO3a的表达呈正相关(r=0.378,P=0.006)。E-cadherin表达与分化程度有关(P<0.05),STMN1表达与肿瘤部位有关(P<0.05),ATP4B表达与T分期有关(P<0.05),三者联合诊断胃癌的ROC曲线下面积为0.915(95%CI:0.859~0.971)。胃癌AGS细胞中E-cadherin过表达后,FOXO3a、ATP4B表达显著升高,AKT1、p-AKT、STMN1表达显著下降。5-FU处理后,E-cadherin过表达组细胞抑制率显著高于对照组,BAX蛋白表达量显著升高。结论E-cadherin、STMN1、ATP4B共同参与胃癌发生,联合检测有助于辅助诊断。Objective To investigate the relationship of E-cadherin,stathmin 1(STMN1)and ATPase H^(+)/K^(+)transporting subunit Beta(ATP4 B)expression in gastric cancer cells,and their clinical significance.Methods The expression of E-cadherin,AKT1,FOXO3 a,STMN1 and ATP4 B were detected by immunohistochemical staining in 51 specimens of gastric cancer tissues and their adjacent tissues,and the correlation between their expression and clinicopathological characteristics were analyzed.Receiver operating characteristic(ROC)curve was used to evaluate the clinical value of E-cadherin,STMN1 and ATP4 B in the joint diagnosis of gastric cancer.E-cadherin-over-expressing gastric cancer AGS cells were constructed by lentivirus-mediated cell transfection,and the protein expression of E-cadherin,FOXO3 a,AKT1,p-AKT,STMN1 and ATP4 B were determined by western blot analysis.Results The positive expression rates of E-cadherin,FOXO3 a,and ATP4 B in gastric cancer tissues were significantly lower than in adjacent tissues(P<0.05),while the positive rates of AKT1 and STMN1 were significantly higher(P<0.05).The expression of E-cadherin was positively correlated with FOXO3 a and ATP4 B(r=0.394,P=0.004;r=0.324,P=0.020).The expression of ATP4 B was positively correlated with FOXO3 a(r=0.378,P=0.006).The positive expression rates of E-cadherin,STMN1,and ATP4 B in gastric cancer tissues were related to tumor grade,tumor location and T stage respectively.The area under ROC curve for the combined diagnosis of E-cadherin,STMN1 and ATP4 B was 0.915(95%CI:0.859-0.971).After over-expression of E-cadherin in AGS cells,the expression of FOXO3 a,ATP4 B increased,and the expression of AKT1,p-AKT and STMN1 decreased with a significant difference.After the treatment with 5-fluorouracil,the inhibition rate of E-cadherin overexpression group was significantly higher than that of the control group,with increased BAX expression.Conclusion E-cadherin,STMN1 and ATP4 B are involved in the occurrence of gastric cancer,and combined detection is helpful to the auxiliary d
