补肾活血通络方+改良PD方案对多发性骨髓瘤肾损害初治患者病情的缓解效果

摘　　要：目的分析补肾活血通络方+改良PD方案(由硼替佐米和地塞米松组成)对多发性骨髓瘤肾损害初治患者病情的缓解效果。方法选择我院2019年1月至2021年3月收治的75例多发性骨髓瘤肾损害初治患者作为研究对象,根据治疗方案差异将其分为对照组(n=36,改良PD方案)和研究组(n=39,补肾活血通络方+改良PD方案)。比较两组的临床疗效、实验室检测指标[血红蛋白、血沉、β_(2)微球蛋白、肌酐、白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)、血管内皮生长因子(VEGF)、表皮细胞生长因子(EGF)、β胶原降解产物(β-CTX)、骨钙素N端中分子片段(N-MID)、骨生长蛋白(BGP)、骨密度(BMD)]、不良反应发生情况。结果研究组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的血红蛋白水平升高,血沉、β_(2)微球蛋白、肌酐水平降低,且研究组优于对照组(P<0.05)。治疗后,两组的IL-1β、IL-8、VEGF、EGF水平均较治疗前显著降低,且研究组低于对照组(P<0.05)。治疗后,两组的β-CTX水平降低,N-MID、BGP、BMD水平升高,且研究组优于对照组(P<0.05)。研究组的不良反应总发生率为2.56%,明显低于对照组的16.67%,差异具有统计学意义(P<0.05)。结论补肾活血通络方联合改良PD方案治疗多发性骨髓瘤肾损害初治患者的效果显著,可有效缓解病情,改善肝肾功能、炎症因子、生长因子及骨代谢指标水平,且不良反应发生率较低。Objective To analyze the alleviating effect of Bushen Huoxue Tongluo recipe combined with modified PD regimen(composed of bortezomib and dexamethasone)on initial treatment multiple myeloma patients with renal damage.Methods Seventy-five initial treatment multiple myeloma patients with renal damage admitted in our hospital from January 2019 to March 2021 were selected as the research objects.According to the difference of treatment schemes,the patients were divided into control group(n=36,modified PD regimen)and study group(n=39,Bushen Huoxue Tongluo recipe+modified PD regimen).The clinical efficacy,laboratory test indexes[hemoglobin,erythrocyte sedimentation rate,β2 microglobulin,creatinine,interleukin-1β(IL-1β),interleukin-8(IL-8),vascular endothelial growth factor(VEGF),epidermal growth factor(EGF),β-C-terminal telopeptide of typeⅠcollagen(β-CTX),N-terminal middle molecular fragment of osteocalcin(N-MID),bone growth protein(BGP),and bone mineral density(BMD)],and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the hemoglobin level of the two groups increased,the erythrocyte sedimentation rate,β_(2) microglobulin and creatinine levels decreased,and those in the study group were better than the control group(P<0.05).After treatment,the levels of IL-1β,IL-8,VEGF and EGF in the two groups were significantly lower than those before treatment,and those in the study group were lower than the control group(P<0.05).After treatment,the levels ofβ-CTX in the two groups decreased,the levels of N-MID,BGP and BMD increased,and those in the study group were better than the control group(P<0.05).The total incidence of adverse reactions in the study group was 2.56%,which was significantly lower than 16.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion Bushen Huoxue Tongluo recipe combined with
