摘 要:目的了解ISO 15189质量管理评审体系对住院患者血常规标本周转时间(turnaround time,TAT)的影响。方法收集评审前后一段时间内(2016年9月vs 2021年9月)天津市中医药研究院附属医院住院患者血常规报告TAT的基本资料,将TAT分段为分析前TAT、准备TAT和检验TAT,并进行数据分析。结果与评审前比较,评审后住院血常规报告分析前TAT的中位数(M)(18 vs 33 min)和第90百分位数(P_(90))(29 vs 53 min)、准备TAT的M(20 vs 39 min)和P_(90)(28 vs 52 min)、检验TAT的M(20 vs 39 min)和P_(90)(28 vs 52 min)均有不同程度的缩短,差异均有统计学意义(均P<0.05)。分析前TAT的分时段统计结果显示,评审后与评审前相比,T1时段M(20 vs 38 min)和P_(90)(28 vs 52 min)、T2时段M(15 vs 31 min)和P_(90)(21 vs 38 min)、T3时段M(10 vs 22 min)和P_(90)(20 vs 28 min)、T4时段M(12 vs 21 min)和P_(90)(22 vs 28 min)、T5时段M(15vs 23 min)和P_(90)(23 vs 26 min)、T6时段M(13 vs 22 min)和P_(90)(21 vs 35 min)均有不同程度的缩短,差异均有统计学意义(均P<0.05)。与评审前相比,评审后血常规M(21 vs 43 min)和P_(90)(32 vs 57 min)、血常规+C-反应蛋白M(33 vs 62 min)和P_(90)(49 vs 87 min)、血常规+C-反应蛋白+血清淀粉样蛋白A M(47 vs 79 min)和P_(90)(65 vs 128 min)均有不同程度的缩短,差异均有统计学意义(均P<0.05)。结论按照ISO 15189质量管理评审体系对检验流程进行持续改进可以缩短血常规报告的TAT。Objective To investigate influence of ISO 15189 quality management evaluation system on the turnaround time(TAT)of blood routine samples of inpatients.Methods The basic TAT data of blood routine report of inpatients in the Affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine for a period of time before and after the review(September 2016 vs September2021)was collected and divide into pre-analysis TAT,preparation TAT and test TAT,and then analyzed.Results The median(M)(18 vs 33 min)and 90th percentile(P_(90))(29 vs 53 min)of pre-analysis TAT,the M(20 vs 39 min)and P_(90)(28 vs 52 min)of preparing TAT,and the M(20 vs 39 min)and P_(90)(28 vs 52 min)of test TAT were shortened to different degrees between the TAT data of September 2016 and September 2021,and the differences were all statistically significant(all P<0.05).The statistical results of time division showed that the M(20 vs 38 min)and P_(90)(28 vs 52 min)of T1,the M(15 vs 31 min)and P_(90)(21 vs 38min)of T2,the M(10 vs 22 min)and P_(90)(20 vs 28 min)of T3,the M(12 vs 21 min)and P_(90)(22 vs 28 min)of T4,the M(15 vs23 min)and P_(90)(23 vs 26 min)of T5,and the M(15 vs 23 min)and P_(90)(23 vs 26 min)of T6 were shortened to different degrees between the pre-analysis TAT data of September 2016 and September 2021,and the differences were all statistically significant(all P<0.05).The M(21 vs 43 min)and P_(90)(32 vs 57 min)of blood routine,the M(33 vs 62 min)and P_(90)(49 vs 87 min)of blood routine+C-reactive protein,and the M(47 vs 79 min)and P_(90)(65 vs 128 min)of blood routine+C-reactive protein+serum amyloid A were shortened to different degrees between the test TAT data of September 2016 and that of September 2021,and the differences were all statistically significant(all P<0.05).Conclusion Continuous improvement of inspection process according to ISO 15189 quality management evaluation system can shorten the TAT of blood routine report.
