摘　　要：目的探讨肾脏少见良性肿瘤的影像学表现。方法回顾性分析28例肾脏少见良性肿瘤患者的临床及影像学资料。结果嗜酸细胞腺瘤(RO)15例,密度均匀,6例病灶中心见瘢痕样密度影,6例出现节段性强化反转。混合型上皮和间质肿瘤(MESTK)5例,1例以实性成分为主,密度及信号均匀,呈轻度强化;4例以囊性成分为主,为多囊样结构,T_(1)WI示病灶以混杂低信号为主,T_(2)WI示病灶以混杂高信号为主,增强扫描分隔及实性成分呈延迟型强化。后肾腺瘤(MA)3例,实性成分为主,钙化多见,T_(1)WI示等及稍低信号,T_(2)WI示等及稍高信号,扩散加权成像(DWI)示高信号,病灶以轻度渐进型强化为主。囊性肾瘤(CN)3例,呈多囊样结构,密度及信号不均,病灶囊壁及病灶内分隔呈轻-中度强化。血管母细胞瘤(RSH)1例,呈渐进性强化并向中心填充。后肾腺纤维瘤(MAF)1例,囊性结构并出血,呈渐进性明显强化。结论肾脏少见良性肿瘤具有一定的影像学特征,有助于术前对肾脏良恶性肿瘤的诊断及鉴别诊断。Objective To evaluate the imaging characteristics of unusual benign renal tumors.Methods The imaging findings of 29 patients with unusual benign renal tumor were studied retrospectively.Results There were 15 cases of oncocytoma,and all of them were homogeneous density and mass.6 cases showed central stellate scars and 6 cases showed segmental enhancement inversions.5 cases of mixed epithelial and stromal tumor(MEST)were included in this study.1 case showed solid lesions with homogeneous density and signal.Slight enhancement was founded on contrast enhanced CT.4 cases were mainly cystic components with multi cystic structures.The lesions were mainly mixed hypo-intense on T_(1)WI and hyper-intense on T_(2)WI.The septum and solid components showed delayed enhancement.The separations and solid masses showed delayed enhancement on three phases.There were 3 cases of metanephric adenoma,and unenhanced CT showed solid mass and calcification.While on MRI,they were equally or slightly low signal intensity on T_(1)WI,equally or slightly high signal intensity on T_(2)WI and high signal intensity on DWI.The lesions were mild progressive enhancement on enhancement scans.3 cases were cystic nephroma and showed multilocular cystic lesions,heterogeneous density and signal.The contrast enhancement scanning displayed light moderate enhancement in the cystic walls and partitions.There was a hemangioblastoma with progressive enhancement and filling to the center.Metanephric adenofibroma appeared as cystic structure with hemorrhage and progressively obvious enhancement.Conclusion There are some special imaging characteristics of unusual benign renal tumors,which could improve their diagnosis and differential diagnosis.
