童年欺凌对心境障碍患者自杀风险的影响:情绪反应的中介作用和冲动性的调节作用

摘　　要：目的探讨情绪反应3方面(情绪敏感性、情绪强度和情绪持续性)在心境障碍患者童年欺凌和自杀风险之间的中介作用和冲动性在童年欺凌和情绪反应3方面之间的调节作用。方法采用一般资料问卷、童年不良经历国际版问卷(ACE-IQ)中的童年欺凌项、冲动性条目、情绪反应性量表(ERS)和自杀行为问卷-修订版(SBQ-R)对496例心境障碍患者进行问卷调查。结果童年欺凌与自杀风险、情绪反应的3方面,即情绪敏感性、情绪强度和情感持续性均呈正相关(P<0.05);自杀风险与情绪敏感性、情绪强度和情绪持续性均呈正相关(P<0.05)。冲动性组和无冲动性组心境障碍患者的童年欺凌发生率比较差异有统计学意义(P<0.05),冲动性组自杀风险、情绪敏感性、情绪强度、情绪持续性得分均高于无冲动性组(P<0.05);情绪反应3方面在童年欺凌和自杀风险之间的中介作用显著,间接效应分别为0.05,0.02和0.03;冲动性可调节童年欺凌与情绪反应3方面之间的关系(P<0.05)。结论心境障碍患者童年欺凌与自杀风险关系密切,情绪反应3方面在童年欺凌与自杀风险关系中起中介作用,而冲动性在童年欺凌和情绪反应3方面之间起调节作用。Objective To explore the mediating effects of emotion sensitivity,intensity and persistence(3 aspects)of emotion reactivity on childhood bullying and suicide risk and the moderating effects of impulsivity on childhood bullying and the 3 aspects of emotion reactivity in patients with mood disorders.Methods A total of 496 patients with mood disorders were selected and assessed with general information questionnaire,childhood bullying items and impulsivity items in Adverse Childhood Experiences-International Questionnaire(ACE-IQ),Emotion Reactivity Scale(ERS)and Suicide Behaviors Questionnaire-Revised(SBQ-R).Results Childhood bullying was positively correlated with suicide risk and emotion sensitivity,intensity and persistence of emotion reactivity(P<0.05).Suicide risk was positively correlated with emotion sensitivity,intensity and persistence(P<0.05).There was significant difference in the incidence rates of childhood bullying between impulsivity group and non-impulsivity group(P<0.05),and the total scores of suicide risk,emotion reactivity in impulsivity group were significantly higher than those in non-impulsivity group(P<0.05).The mediating effect of the 3 aspects of emotion reactivity on childhood bullying and suicide risk was significant and the indirect effect was 0.05,0.02 and 0.03,respectively.Impulsivity moderated the relationship between childhood bullying and the 3 aspects of emotion reactivity(P<0.05).Conclusion There is a close relationship between childhood bullying and suicide risk in patients with mood disorders.Emotion sensitivity,intensity and persistence play a mediating role in the relationship between childhood bullying and suicide risk,while impulsivity plays a moderating role in the relationship between childhood bullying and emotion reactivity.
