摘 要:目的:探讨游离脂肪酸受体4(FFAR4)、肝癌衍生生长因子(HDGF)在胆管癌组织中的表达及其与临床病理特征和预后的关系。方法:将80例具有完整生存资料的胆管癌组织和40例胆管癌根治性术的癌旁胆管组织分别作为胆管癌组织(n=80)、癌旁组织(n=40);采用免疫组化检测FFAR4、HDGF表达水平,分析其与临床病理及预后的关系。结果:FFAR4、HDGF蛋白在胆管癌组织高表达率均高于癌旁组织(65.00%vs.17.50%;83.75%vs.27.50%,P<0.05);不同性别、年龄、肿瘤部位、肿瘤大小、神经侵犯情况胆管癌组织中FFAR4、HDGF蛋白表达水平比较,差异无统计学意义(P>0.05);低分化、临床分期Ⅲ-Ⅳ期、发生淋巴结转移患者胆管癌组织中FFAR4、HDGF蛋白表达水平均分别高于中高分化、临床分期Ⅰ-Ⅱ期、未发生淋巴结转移患者,差异均有统计学意义(P<0.05);Logisitic回归分析显示临床分期高、分化程度低、发生淋巴结转移是FFAR4及HDGF蛋白高表达的独立危险因素(P<0.05);随访两年,FFAR4及HDGF蛋白高表达患者两年总生存率50.00%(26/52)、55.22%(37/67)低于低表达患者的78.57%(22/28)、84.62%(11/13),FFAR4蛋白、HDGF蛋白高表达患者两年无进展生存率为42.31%(22/52)、47.76%(32/67),低于低表达患者的71.73%(20/28)、76.92%(10/13),差异均有统计学意义(P<0.05)。结论:胆管癌组织FFAR4、HDGF高表达与肿瘤分期、分化程度、淋巴结转移相关,且可预示不良预后。Objective:To investigate the expression of free fatty acid receptor 4(FFAR4)and hepatoma-derived growth factor(HDGF)in cholangiocarcinoma and their relationship with clinicopathological features and prognosis.Methods:80 cases of cholangiocarcinoma with complete survival data and 40 cases of paracancerous bile duct tissues who underwent radical operation for cholangiocarcinoma were selected as cholangiocarcinoma tissues(n=80)and adjacent tissues(n=40).The expression levels of FFAR4 and HDGF were detected by immunohistochemistry,and the relationship with clinicopathology and prognosis was analyzed.Results:The high expression rates of FFAR4 protein and HDGF protein in cholangiocarcinoma tissues were 65.00%and 83.75%,respectively,which were higher than those in adjacent tissues(17.50%&27.50%,P<0.05).There was no significant difference in the expression levels of FFAR4 and HDGF in cholangiocarcinoma tissues with different gender,age,tumor location,tumor size and nerve invasion(P>0.05).The expression levels of FFAR4 and HDGF protein in cholangiocarcinoma tissues of patients with low differentiation,clinical stageⅢ-Ⅳ,and lymph node metastasis were higher than those of patients with medium and high differentiation,clinical stageⅠ-Ⅱ,and no lymph node metastasis(P<0.05).Logisitic regression analysis showed that high clinical stage,low degree of differentiation and lymph node metastasis were the influencing factors of high expression of FFAR4 and HDGF protein.After 2-year follow-up,the 2-year overall survival rate of patients with high expression of FFAR4 protein and HDGF protein was 50.00%(26/52)and 55.22%(37/67),which was lower than 78.57%(22/28)and 84.62%(11/13)of patients with low expression.The 2-year progression-free survival rate of patients with high expression of FFAR4 protein and HDGF protein was 42.31%(22/52)and 47.76%(32/67),which was lower than 71.73%(20/28)and 76.92%(10/13)of patients with low expression(P<0.05).Conclusion:FFAR4 and HDGF are highly expressed in cholangiocarcinoma,and their high express
