精神分裂症患者自杀未遂与甲状腺激素关联分析

摘　　要：目的:探讨精神分裂症患者自杀未遂与甲状腺激素之间的关系。方法:通过横断面调查符合《国际疾病分类》第10版(ICD-10)诊断的精神分裂症患者832例,根据患者既往是否有自杀未遂史,分为自杀未遂组(n=99)和非自杀未遂组(n=733)。收集两组社会人口学资料,对临床总体印象量表(CGI)及甲状腺激素各指标水平进行评估与检测。采用多因素二项Logistic回归分析精神分裂症患者自杀未遂的影响因素。结果:自杀未遂组促甲状腺激素(TSH)(Z=2.285,P=0.022)、游离三碘甲状原氨酸(FT3)(t=2.177,P=0.031)、三碘甲状原氨酸(T3)(t=2.045,P=0.042)水平低于非自杀未遂组。多因素Logistic回归分析显示,FT3为自杀未遂发生的影响因素(OR=0.506,P=0.030);血清FT3浓度越低,自杀风险越高。结论:伴有自杀未遂的精神分裂症患者存在甲状腺激素功能紊乱。Objective:To explore the relationship between thyroid hormone and suicide attempt in patients with schizophrenia.Method:A cross-sectional survey were conducted in 832 patients with schizophrenia who met the diagnosis of the International Classification of Diseases(ICD-10).They were divided into suicide attempt group(n=99)and non-suicide attempt group(n=733)according to whether they had a history of suicide attempts.The social demographic data were collected in the two groups.Clinical Global Impression Scale(CGI)and the index levels of thyroid hormone were evaluated and measured.Multivariate binomial Logistic regression was used to analyze the influencing factors of suicide attempt in patients with schizophrenia.Results:The levels of thyrotropin(TSH)(Z=2.285,P=0.022),free triiodothyronine(FT3)(t=2.177,P=0.031)and triiodothyronine(T3)(t=2.045,P=0.042)in the suicide attempt group were lower than those in the non-suicide attempt group.Multivariate Logistic regression analysis showed that FT3 was influencing factor with suicide attempt(OR=0.506,P=0.030),and the lower the plasma FT3 concentration,the higher the risk of suicide.Conclusion:Thyroid hormone dysfunction exists in schizophrenic patients with suicide attempt.
