伴与不伴有精神病性症状的青少年抑郁症患者甲状腺激素水平比较

摘　　要：目的:比较青少年抑郁症患者伴与不伴有精神病性症状的甲状腺激素水平,探讨甲状腺激素对鉴别是否伴有精神病性症状的应用价值。方法:将75例伴有精神病性症状的青少年抑郁患者作为研究组,76例不伴有精神病性症状的青少年抑郁患者作为对照组,比较两组血清样本的促甲状腺激素(TSH)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)和游离甲状腺素(FT4)水平。采用受试者工作曲线(ROC)评价T3单独鉴别伴有精神病性症状的青少年抑郁的效能。结果:研究组T3水平明显高于对照组;研究组女性患者的T3水平为1.69(1.50,1.94)nmol/L,高于对照组女性患者[1.59(1.43,1.74)nmol/L],差异具有统计学意义(P<0.05)。两组男性患者的甲状腺激素水平差异无统计学意义(P>0.05)。T3对鉴别伴有精神病性症状的女性青少年抑郁症的诊断效能:曲线下面积(AUC)为0.627,诊断效能较低。结论:女性青少年抑郁症患者的T3水平可能与精神病性症状相关。Objective:To compare the levels of thyroid hormones in adolescent depressive patients with and without psychotic symptoms,and explore the application value of thyroid hormones in distinguishing psychotic symptoms.Method:75 depressive adolescent patients with psychotic symptoms were selected as the study group and 76 depressive adolescent patients without psychotic symptoms as the control group.The serum levels of thyrotropin(TSH),triiodothyronine(T3),thyroxine(T4),free triiodothyronine(FT3)and free thyroxine(FT4)were compared between the two groups.The receiver operating curve(ROC)was used to evaluate the efficacy of T3 in differentiating depression in adolescents with psychotic symptoms.Results:The T3 level in the study group was higher than that in the control group.The T3 level of female patients in the study group was 1.69(1.50,1.94)nmol/L,which was significantly higher than that of female patients in the control group[1.59(1.43,1.74)nmol/L],the difference was statistically significant(P<0.05).There was no significant difference in the level of thyroid hormone between male patients of two groups(P>0.05).The diagnostic efficacy of T3 in differential diagnosis of depression in female adolescents with psychotic symptoms:the area under the curve(AUC)was 0.627,and the diagnostic efficiency was low.Conclusion:The level of T3 in female adolescent patients with depression may be correlated with psychotic symptoms.
