参芪固本方破壁饮片对大肠癌化疗后患者造血功能改善情况及量效关系研究

摘　　要：目的:探究参芪固本方破壁饮片对大肠癌化疗后患者造血功能的改善情况及量效关系。方法:选取孝感市妇幼保健院于2019年9月-2020年12月收治的60例大肠癌化疗后出现造血抑制的患者为研究对象,随机分为A组(20例)、B组(20例)和观察组(20例),A组采取参芪固本方传统饮片煎剂,B组采取中成药贞芪扶正颗粒,观察组则采取参芪固本方破壁饮片,观察并对比两组患者的治疗前后的造血功能、生活质量及中医证候积分。结果:治疗前三组患者的白细胞计数(White Blood Cell Count,WBC)、中性粒细胞(Neutrophil,NC)、血红蛋白(Hemoglobin,Hb)、红细胞(Red Blood Cell,RBC)、血小板计数(Platelet Count,PLT)差异均不显著,具有可比性,治疗后观察组患者的WBC和NC均较A、B组显著提高,差异具有统计学意义(P <0.05),而三组治疗后Hb、RBC及PLT均较治疗前有所提高,三组间差异不显著(P> 0.05);通过对比三组患者治疗后的Karnofsky功能状态评分法积分评价三组患者的临床疗效,观察组的总有效率与A组、B组的差异均不具有统计学意义(P> 0.05);治疗后观察组的中医证候积分显著低于A、B组,差异显著(P <0.05)。结论:参芪固本方破壁饮片对大肠癌化疗后患者的造血功能及生活质量具有一定的改善效果,与贞芪扶正颗粒与传统饮片相比,能够更好地缓解临床症状。Objective:To explore the effect of wall breaking decoction pieces of the Shenqi Guben decoction(参芪固本方破壁饮片)on the improvement of hematopoietic function in patients with colorectal cancer after chemotherapy and the dose-effect relationship.Methods:A total of 60 patients with hematopoietic inhibition after chemotherapy for colorectal cancer in Xiaogan Maternal and Child Health Hospital from September 2019 to December 2020 were selected as the research subjects and randomly divided into group A(20 cases),group B(20 cases)and the observation group(20 cases).Group A was treated with traditional decoction of the Shenqi Guben prescription,and group B was treated with Chinese patent medicine,the Zhenqi Fuzheng granules(贞芪扶正颗粒).The observation group was treated with wall breaking decoction pieces of the Shenqi Guben prescription,and the hematopoietic function,quality of life and TCM syndrome scores before and after treatment were observed and compared between the two groups.Results:There were no significant differences in WBC,NC,Hb,RBC and PLT among the three groups before treatment,which were comparable.After treatment,WBC and NC in the observation group were significantly increased compared with those in group A and B,the difference was of statistical significance(P<0.05).After treatment,Hb,RBC and PLT in the three groups were all increased,but there was no significant difference among the three groups(P>0.05).The Karnofsky scores in the three groups after treatment were compared to evaluate the clinical efficacy in the three groups.The total effective rate in the observation group was not significantly different from group A and group B(P>0.05).The TCM syndrome scores in the observation group were significantly lower than group A and group B after treatment,the difference was of statistical significance(P<0.05).Conclusion:Wall breaking decoction pieces of the Shenqi Guben prescription can improve the hematopoietic function and quality of life of patients with colorectal cancer after chemoth
