膀胱副神经节瘤的临床特点分析与治疗

摘　　要：目的分析膀胱副神经节瘤的临床特点,总结疾病特征,提高对膀胱副神经节瘤的早发现、早诊断、早准备及早治疗的能力。方法回顾性分析2014至2021年间北京大学深圳医院泌尿外科收治的7例膀胱副神经节瘤患者的临床资料、诊断、治疗经过及术后病理资料。结果膀胱副神经节瘤是一种罕见疾病,典型表现为排尿后出现血压升高、头痛、心悸等症状,CT增强检查发现肿物强化明显且均匀,血儿茶酚胺、血浆甲氧基肾上腺素类似物、24小时尿儿茶酚胺以及24小时尿香草扁桃酸有不同程度升高,手术切除是最主要的治疗方法,诊断膀胱副神经节瘤的病理免疫组化标记物主要有CgA、Syn、CD56及S-100。结论膀胱副神经节瘤的临床表现可分为症状型和无功能型,临床上对于症状型患者,结合影像学检查、儿茶酚胺及其代谢产物化验,诊断较为容易;而对于无功能型的患者,当影像学发现膀胱肿物且CT增强显示肿物强化明显且均匀时,临床医生应考虑到膀胱副神经节瘤的可能。Objective To analyze the clinical characteristics of bladder paraganglioma,summarize the disease characteristics,and improve the ability of early detection,early diagnosis,early preparation and early treatment of bladder paraganglioma.Methods The clinical data,diagnosis,treatment process and postoperative pathological data of 7 patients with bladder paraganglioma admitted to the Department of Urology,Peking University Shenzhen Hospital from 2014 to 2021 were retrospectively analyzed.Results Bladder paraganglioma is a rare disease,typically manifested as increased blood pressure,headache,palpitations and other symptoms after urination.CT enhanced examination revealed obvious and uniform tumor enhancement.Blood catecholamines,plasma metanephrine analogs,the 24-hour urinary catecholamines and 24-hour urinary vanillylmandelic acid were increased to varying degrees.Surgical resection is the main treatment method.The pathological immunohistochemical markers for the diagnosis of bladder paraganglioma are mainly CgA,Syn,CD56 and S-100.Conclusion The clinical manifestations of bladder paraganglioma can be divided into symptomatic and asymptomatic.Clinically,for symptomatic patients,it is easier to diagnose by combining imaging examinations,results of catecholamines and their metabolites.For asymptomatic patients,when a bladder tumor is found on imaging and CT-enhanced shows obvious and uniform tumor enhancement,clinicians should consider the possibility of bladder paraganglioma.
