摘 要:目的 探讨Silva分型在子宫颈腺癌中的病理特征及临床意义。方法 回顾性分析2013—2020年南方医科大学附属佛山市妇幼保健院收治的宫颈腺上皮病变35例患者,包括宫颈原位腺癌11例,宫颈浸润性腺癌(普通型)24例。根据Silva分型标准重新分型,分析与临床病理特征的相关性。结果 子宫颈腺癌Silva分型与临床分期具有相关性(P<0.001)。Sliva C型肿瘤厚度明显高于Sliva A型(P<0.001)与Sliva B型(P<0.05),Sliva B型肿瘤厚度明显高于Sliva A型(P<0.001)。Sliva C肿瘤淋巴脉管累及、神经侵犯明显高于Sliva A与Sliva B(P<0.05)。结论 Silva分型通过肿瘤生长方式评估肿瘤的生物学行为,可以预测患者的预后并完善治疗策略。Objective To investigate the pathological features and clinical significance of Silva pattern system in cervical adenocarcinoma. Methods Thirty-five cases diagnosed as cervical glandular epithelium lesions were selected from 2013 to 2020 in the Maternal and Children’s Hospital of Foshan, including 11 cases of adenocarcinoma in situ and 24 cases of invasive adenocarcinoma(usual type) of the cervix. The correlation between Silva pattern system and clinicopathological features was analyzed. Results The Silva pattern system demonstrated relevance for clinical stage(P<0.001). The tumor thickness of Sliva C was significantly higher than that of Sliva A(P<0.001) and Sliva B(P<0.05). The tumor thickness of Sliva B was significantly higher than that of Sliva A(P<0.001). The lymphatic vascular involvement and nerve invasion of Sliva C were significantly more than that of Sliva A and Sliva B(P<0.05). Conclusion The Silva pattern system can be used to evaluate the biological behavior of cervical adenocarcinoma by its growth pattern, and to predict the prognosis and perfect treatment strategies of the patients.
