新辅助化疗联合腹腔镜远端胃癌根治术治疗进展期胃癌患者的临床价值

摘　　要：目的探讨新辅助化疗联合腹腔镜远端胃癌根治术治疗进展期胃癌患者的临床价值。方法选取本院收治的74例进展期胃癌患者,采用红蓝双色球法将其分为两组,各37例,对照组采用腹腔镜远端胃癌根治术治疗,观察组采用新辅助化疗联合腹腔镜远端胃癌根治术,治疗后1个月比较两组的治疗效果,并统计不良反应发生率、围手术期营养指标、围手术期临床指标、生存质量。结果观察组总有效率、疾病控制率分别为56.76%、78.38%,高于对照组的32.43%、56.76%,差异有统计学意义(P<0.05);观察组贫血、白细胞减少、口腔黏膜炎、恶心呕吐、肝功能异常发生率与对照组对比,差异无统计学意义(P>0.05);观察组围手术期营养指标中血清白蛋白(ALB)、前白蛋白(PA)、血红蛋白(Hb)水平均高于对照组,差异有统计学意义(P<0.05);观察组围手术期临床指标中手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组世界卫生组织生存质量测定量表(WHOQOL)评分高于对照组,差异有统计学意义(P<0.05)。结论新辅助化疗联合腹腔镜远端胃癌根治术治疗进展期胃癌患者能够有效提高治疗效果,安全可行。Objective To investigate the clinical value of neoadjuvant chemotherapy combined with laparoscopic distal radical gastrectomy in the treatment of advanced gastric cancer.Methods Seventy-four patients with advanced gastric cancer admitted to our hospital were selected and divided into two groups with 37 cases in each group by red and blue dual-color ball method.The control group was treated with laparoscopic radical gastrectomy for distal gastric cancer,and the observation group was treated with neoadjuvant chemotherapy combined with laparoscopic radical gastrectomy for distal gastric cancer.One month after treatment,the therapeutic effects of the two groups were compared.The incidence of adverse reactions,perioperative nutritional indicators,perioperative clinical indicators and quality of life were analyzed.Results The total effective rate and disease control rate(56.76%,78.38%)in the observation group were higher than those in the control group(32.43% and 56.76%),the difference was statistically significant(P<0.05).There were no significant differences in the incidence of anemia,leucopenia,oral mucitis,nausea and vomiting,and abnormal liver function between the observation group and the control group(P>0.05).The levels of serum albumin(ALB),prealbumin(PA)and hemoglobin(Hb)in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The perioperative clinical indexes of the observation group were lower than those of the control group,the difference was statistically significant(P<0.05).The World Health Organization quality of life scale(WHOQOL)in observation group was higher than that in control group,the difference was statistically significant(P<0.05).Conclusion Neoadjuvant chemotherapy combined with laparoscopic distal radical gastrectomy for advanced gastric cancer can effectively improve the therapeutic effect,safe and feasible.
