摘 要:目的采用锥形束CT(Cone Beam CT,CBCT)测量观察青少年颞下颌关节骨关节病患者异常影像形态,分析相关影响因素。方法选择2019年10月至2021年9月医院口腔科收治的经CBCT检查诊断为颞下颌关节骨关节病的65例青少年患者为试验组,另选择同一时间段经CBCT检查无颞下颌关节骨关节病症状的65例青少年为对照组。比较两组的CBCT影像学成像参数差异,并对异常影像形态的相关影响因素进行分析。结果试验组颞下颌关节左侧的后间隙、上间隙等CBCT测量指标与右侧比较差异有统计学意义(P<0.05);对照组左侧与右侧各测量指标比较差异不明显(P>0.05)。相较于对照组,试验组CBCT测量的上间隙、后间隙,以及前后径、内外径指标均有下降,口腔不良习惯发生率明显升高(均P<0.05)。结论青少年颞下颌关节骨关节病与髁突异常形态相关,CBCT能较好地反映髁突位置及形态变化,为临床诊治提供科学依据。Objective To analyze the relevant influencing factors by observing and measuring the abnormal morphology of imaging obtained from adolescent patients with temporomandibular joint osteoarthropathy(TMJ-OA)by using cone beam CT(CBCT).Methods A total of 65 adolescents patients with TMJ-OA diagnosed by CBCT from October 2019 to September 2021 in our hospital were selected as the test group,and another 65 adolescents patients without TMJ-OA diagnosed by CBCT during the same time period were selected as the control group.The differences of imaging parameters on CBCT images between the two groups were statistically compared,and the relevant influencing factors of abnormal imaging morphology were analyzed.Results CBCT imaging parameters such as posterior and superior spaces on the left side of temporomandibular joint(TMJ)in the test group were significantly different from those on the right side(P<0.05).There was no significant difference for each imaging parameter between the left and right sides of TMJ in the control group(P>0.05).Compared with the control group,the parameter of superior and posterior space,as well as the anterior and posterior,inside and outside diameter indicators measured by CBCT were decreased in the test group,and the incidence of poor oral habits also increased significantly(all P<0.05).Conclusion Adolescent TMJ-OA is associated with abnormal condylar morphology.CBCT can better reflect the position and morphological changes of condylar,and provide scientific basis for clinical diagnosis and treatment.
