摘　　要：目的不同错配修复蛋白(MMR)检测组合对子宫内膜癌预后判断的价值及经济学意义。方法通过免疫组化方法检测子宫内膜癌组织中MMR的表达,按照MMR不同检测组合进行分组,将检测MLH1、MSH2定义为A组,检测MSH6、PMS2定义为B组,检测MLH1、MSH2、MSH6和PMS2定义为C组,根据各蛋白阳性结果不同,将患者分为微卫星稳定性(MSS)与微卫星不稳定性(MSI),分析各组MSI与患者临床病理特征及雌激素受体(ER)、孕激素受体(PR)的关系,评估不同MMR检测组合对子宫内膜癌患者预后预测效果。结果A组患者MSI与ER、PR的表达有关(χ^(2)=7.587、6.935,P<0.05);B组患者MSI与肿瘤组织分化程度、ER、PR的表达有关(χ^(2)=6.566~8.765,P<0.05);C组患者MSI与肿瘤组织分化程度、ER、PR的表达有关(χ^(2)=6.774~11.600,P<0.05)。结论通过对4种MMR中两种MMR的检测,也可对子宫内膜癌患者的预后进行判断,此种方法具有一定的临床意义和经济价值。Objective To investigate the value and economic significance of combined measurement of different mismatch repair proteins(MMRs)in the prognostic evaluation of endometrial carcinoma.Methods Immunohistochemistry was used to measure the expression of MMRs in endometrial carcinoma tissue,and according to the combination of MMRs for measurement,the tissue samples were divided into group A(measurement of MLH1 and MSH2),group B(measurement of MSH6 and PMS2),and group C(measurement of MLH1,MSH2,MSH6,and PMS2).According to the positive results of each protein,the patients were divided into microsatellite stability(MSS)subgroup and microsatellite instability(MSI)subgroup.The association of MSI with clinicopathological features,estrogen receptor(ER),and progesterone receptor(PR)was analyzed for each group,and the effect of combined measurement of different MMRs in predicting the prognosis of patients with endometrial carcinoma was evaluated.Results In group A,MSI was associated with the expression of ER and PR(χ^(2)=7.587,6.935,P<0.05);in group B,MSI was associated with the degree of tumor differentiation,with the expression of ER and PR(χ^(2)=6.566-8.765,P<0.05);in group C,MSI was associated with the degree of tumor differentiation,with the expression of ER and PR(χ^(2)=6.774-11.600,P<0.05).Conclusion Combined measurement of two MMRs may help to evaluate the prognosis of patients with endometrial carcinoma,with certain clinical significance and economic value.
