双重血浆分子吸附系统结合低置换量血浆置换术在肝衰竭患者中的应用分析

摘　　要：目的:探讨双重血浆分子吸附系统(DPMAS)结合低置换量血浆置换术(LPE)在肝衰竭患者中的应用效果。方法:选取2019年2月-2021年2月于赣州市第五人民医院收治的80例肝衰竭患者,根据治疗方法不同分为对照组和观察组,各40例。对照组采用LPE治疗,观察组采用DPMAS+LPE治疗。对比两组治疗前后炎症介质水平和肝功能、凝血功能、免疫功能指标。结果:治疗前,两组各项肝功能、凝血功能、免疫功能指标和各炎症介质水平比较,差异均无统计学意义(P>0.05);治疗后,观察组丙氨酸氨基转移酶(ALT)、血清总胆红素(TBIL)、间接胆红素(DBIL)、白蛋白(ALB)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-10(IL-10)、凝血酶原时间(PT)、CD8^(+)水平均低于对照组,治疗总有效率、凝血酶原活动度(PTA)、CD4^(+)水平均高于对照组,差异均有统计学意义(P<0.05)。结论:DPMAS结合PE可提升肝衰竭患者的治疗效果,改善凝血、免疫功能,缓解症状,促进肝功能恢复,值得推广。Objective:To investigate the effect of DPMAS(double plasma molecular adsorption system) combined with low exchange volume plasma exchange(LPE) in patients with liver failure.Method:A total of 80patients with liver failure admitted to Ganzhou Fifth People’s Hospital from February 2019 to February 2021 were selected.They were divided into control group and observation group according to different treatment methods,with 40 cases in each group.The control group was treated with LPE,and the observation group was treated with DPMAS+LPE.The levels of inflammatory mediators and liver function,coagulation function and immune function indexes were compared between the two groups before and after treatment.Result:Before treatment,there were no significant differences in liver function,coagulation function,immune function indexes and levels of inflammatory mediators between two groups(P>0.05).After treatment,alanine aminotransferase(ALT),serum total bilirubin(TBIL),indirect bilirubin(DBIL),albumin(ALB),C reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-10(IL-10),prothrombin time(PT),CD8^(+) levels of the observation group were lower than those of control group,the total effective rate,prothrombin activity(PTA) and CD4^(+) levels were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:DPMAS combined with PE can improve the therapeutic effect of liver failure patients,improve coagulation and immune function,relieve symptoms,and promote liver function recovery,which is worthy of promotion.
