摘　　要：目的:探讨内镜下密集套扎(EVL)配合胃底曲张静脉组织胶注射(TAI)方案用于食管胃底静脉曲张(esophagogastric varices,EGV)的效果。方法:选取2020年6月-2021年6月在荆州市第二人民医院就诊的80例食管胃底静脉曲张患者为研究对象,将患者按照随机数字表法分为对照组和观察组,每组40例。对照组采用EVL治疗,观察组采用EVL配合TAI方案治疗。分析两组的治疗效果。结果:观察组患者静脉曲张消除率及治疗总有效率均高于对照组(P<0.05)。术后1个月,两组门静脉及脾静脉血流速度比较,差异均有统计学意义(P<0.05),但两组胃左静脉血流速度比较,差异无统计学意义(P>0.05)。术后6个月,观察组胃底曲张静脉直径明显低于对照组(P<0.05)。观察组患者再出血率明显低于对照组(P<0.05),两组术后并发症比较,差异无统计学意义(P>0.05)。结论:针对食管胃底静脉曲张患者实施EVL联合TAI方案治疗的效果理想,可促进患者静脉曲张症状缓解,且可降低再出血风险,值得推广。Objective:To explore the efficacy of esophagogastric varices (EGV) with endoscopic varicose vein ligation (EVL) and gastric varicose vein tissue adhesive injection (TAI).Method:A total of 80 patients with esophageal and gastric varices in Jingzhou Second People’s Hospital from June 2020 to June 2021 were selected as the research subjects,the patients were divided into control group and observation group according to random number table method,40 cases in each group.The control group was treated with EVL,and the observation group was treated with EVL combined with TAI.The therapeutic effects of the two groups were analyzed.Result:The varicose elimination rate and total effective rate in observation group were higher than those in control group (P<0.05).1 month after surgery,comparison of blood flow velocity of portal vein and splenic vein between the two groups,the differences were statistically significant (P<0.05),however,there was no significant difference in the blood flow velocity of left gastric vein between the two groups (P>0.05).6 months after surgery,the diameter of fundus varices in the observation group was significantly lower than that in the control group (P<0.05).The rebleeding rate in observation group was significantly lower than that in control group (P<0.05).There was no significant difference in postoperative complications between the two groups (P>0.05).Conclusion:The effect of EVL combined with TAI in the treatment of patients with esophageal and gastric varices is ideal,which can promote the relief of variceal symptoms and reduce the risk of rebleeding,worth popularizing.
