单纯根管治疗与根管结合手术治疗前牙根尖周囊肿的效果对比

摘　　要：目的:比较单纯根管治疗与根管结合手术治疗前牙根尖周囊肿的效果。方法:选取2018年1月-2020年1月于上饶市人民医院进行治疗的前牙根尖周囊肿患者120例,收集临床资料开展本研究,依照治疗方案的差异分为对照组和研究组,每组60例。对照组采用单纯根管治疗,研究组采用根管治疗结合手术干预。对比两组临床治疗效果、术后并发症发生情况、治疗满意度、门诊治疗费用、康复用时、术后疼痛程度及炎症水平。结果:研究组总有效率高于对照组(P<0.05);术后8周,研究组并发症发生率较对照组更低(P<0.05);研究组治疗满意度评分及门诊治疗费用均高于对照组,康复用时短于对照组,术后疼痛程度低于对照组,差异均有统计学意义(P<0.05);术前两组炎症水平比较,差异均无统计学意义(P>0.05),术后6周,研究组IL-6、CRP、TNF-α水平均低于对照组(P<0.05)。结论:对前牙根尖周囊肿患者实施根管治疗结合手术干预,可有效提升疗效,还能降低术后并发症发生率及炎症水平,提高患者满意度,促进康复,值得借鉴。Objective:To compare the effect of root canal therapy alone and root canal combined with surgery in the treatment of periapical cyst of anterior teeth.Method:A total of 120 patients with periapical cyst of anterior teeth treated in Shangrao People’s Hospital from January 2018 to January 2020 were selected.The clinical data were collected to carry out this study.They were divided into control group and study group according to the difference of treatment scheme,60 cases in each group.The control group was treated with root canal therapy alone,and the study group was treated with root canal therapy combined with surgical intervention.Clinical treatment effect,postoperative complications,treatment satisfaction,outpatient treatment cost,rehabilitation time,postoperative pain degree and level of inflammation were compared between the two groups.Result:The total effective rate of the study group was higher than that of the control group (P<0.05);the incidence of complications in the study group was lower than that in the control group 8 weeks after operation (P<0.05);the treatment satisfaction score and outpatient treatment cost of the study group were higher than those of the control group,rehabilitation time was shorter than that of the control group,postoperative pain was lower than that of the control group,the differences between the two groups were significant (P<0.05);there were no differences in inflammation levels between the two groups before operation (P>0.05),6 weeks after operation,the levels of IL-6,CRP and TNF-α in the study group were lower than those in the control group (P<0.05).Conclusion:Root canal therapy combined with surgical intervention in the treatment of periapical cyst of anterior teeth can effectively improve the curative effect,reduce the incidence of postoperative complications and inflammatory level,improve satisfaction of patient and promote rehabilitation,which is worthy of reference.
