摘 要:目的:研究粪菌移植(FMT)对酒精性肝损伤(ALI)大鼠肠黏膜屏障功能及肝功能的影响。方法:2021年4-6月将30只雄性SD大鼠随机分为正常对照组(C组)、酒精模型组(M组)和FMT治疗组(T组),每组10只。C组给予等容量蒸馏水灌胃,基础饲料饲养;M组、T组按10 mL/kg,给予56°白酒灌胃,持续8周,第5周开始,T组给予粪菌液灌胃,C组及M组均给予生理盐水。测定并比较各组大鼠的生理学指标(体重、肝指数)、血液相关指标[D-乳酸(D-LA)、血浆内毒素水平(ET)、肿瘤坏死因子-α(TNF-α)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)],对肝组织进行HE染色,观察光镜下肝组织病理变化,并对其进行病理诊断与Scheuer评分。结果:第2、7、8周,与C组相比,M组与T组大鼠的体重均显著降低(P<0.05);第7、8周,T组体重均显著高于M组(P<0.05)。与C组相比,M组与T组大鼠的肝指数、D-LA、ET、TNF-α、ALT、AST均显著升高(P<0.05),且T组均显著低于M组(P<0.05)。HE染色结果显示,C组大鼠肝细胞无损伤,M组与T组大鼠肝细胞发生损伤,且T组大鼠肝细胞损伤程度低于M组。三组炎症活动度分级和纤维化程度分期比较,差异均有统计学意义(P<0.05);其中M组的炎症活动度分级与纤维化程度分期均高于T组与C组(P<0.05),而T组与C组炎症活动度分级与纤维化程度分期比较,差异均无统计学意义(P>0.05)。结论:FMT可以有效保护大鼠肠黏膜屏障功能,缓解炎症,改善肝功能,对酒精导致的大鼠肝损伤具有并保护作用。Objective:To study the effects of fecal microbiota transplantation (FMT) on intestinal mucosal barrier function and liver function in rats with alcoholic liver injury (ALI).Method:From April to June 2021,30 male SD rats were randomly divided into normal control group (group C),alcohol model group (group M) and FMT treatment group (group T),with 10 rats in each group.Group C was given equal volume distilled water by intragastric administration and basal feed;group M and group T were given 10 mL/kg of 56° liquor intragastric administration for 8 weeks,from the 5th week,group T was given fecal bacteria solution intragastric administration,and group C and group M were given normal saline.Physiological indexes (body weight,liver index) and blood related indexes (D-LA,ET,TNF-α,ALT,AST) were measured and compared,and liver tissue was stained with HE,the pathological changes of liver tissue under light microscope were observed,and the pathological diagnosis and Scheuer score were performed.Result:At the 2th,7th and 8th weeks,compared with group C,the weight of rats in group M and group T decreased significantly (P<0.05);at the 7th and 8th weeks,the weight of group T were significantly higher than that of group M (P<0.05).Compared with group C,the liver index,D-LA,ET,TNF-α,ALT and AST in group M and group T were significantly higher (P<0.05),and those in group T were significantly lower than those in group M (P<0.05).The results of HE staining showed that there was no damage to hepatocytes in group C,and hepatocytes were injured in group M and group T,and the degree of hepatocyte damage in group T was lower than that in group M.There were statistically significant differences in inflammatory activity grade and fibrosis stage among the three groups (P<0.05);the inflammation activity grade and fibrosis stage of group M were higher than those of group T and group C (P<0.05),and there were no significant differences in inflammatory activity grade and fibrosis stage between group T and group C (P>0.05).Conclusion:FMT can ef
