多层螺旋CT动态增强及后处理技术在结肠癌分期诊断中的临床应用价值

摘　　要：目的:探究多层螺旋CT(MSCT)动态增强及后处理技术在结肠癌分期诊断中的临床应用价值。方法:选取2018年5月-2020年5月九江市第一人民医院85例结肠癌患者作为研究对象,所有患者均行MSCT动态增强扫描及多模式图像后处理,对影像学特征予以观察,评估结肠癌分期情况,并与术后病理学诊断结果进行比较,分析MSCT术前诊断结肠癌分期的敏感度、特异度及准确率。结果:85例患者中,病变位于升结肠31例,降结肠28例,盲肠16例,横结肠10例;病变形态主要包括溃疡型53例、肿块型22例、浸润型10例;MSCT术前诊断≤T_(2)期、T_(3)期、T_(4)期、N_(0)期、N_(1)期、N_(2)期准确率分别为97.65%、86.73%、91.13%、89.41%、75.00%、79.45%,与术后病理诊断一致性高(Kappa=0.619、0.714)。结论:MSCT动态增强及后处理技术可帮助结肠癌术前病理分期,准确率高,与术后病理诊断结果一致性高,具有较高的临床价值。Objective:To explore the clinical value of dynamic enhancement and post-treatment technology of multi-slice spiral CT (MSCT) in the staging diagnosis of colon cancer.Method:A total of 85 patients with colon cancer in Jiujiang NO.1 People’s Hospital from May 2018 to May 2020 were selected as the research objects.All patients were given MSCT dynamic enhancement scan and multi-mode image post-treatment,the imaging features were observed,the staging of colon cancer was evaluated,and the results were compared with those of postoperative pathological diagnosis.The sensitivity,specificity and accuracy of MSCT in preoperative diagnosis of colon cancer staging were analyzed.Result:The lesions of 85 patients were located in ascending colon (31 cases),descending colon (28 cases),cecum (16 cases) and transverse colon (10 cases).There were 53 cases of ulcerative type,22 cases of mass type and 10 cases of infiltration type;the accuracy of MSCT in preoperative diagnosis of ≤T_(2),T_(3),T_(4),N_(0),N_(1) and N_(2) stage were 97.65%,86.73%,91.13%,89.41%,75.00% and 79.45%,respectively,which was highly consistent with postoperative pathological diagnosis (Kappa=0.619,0.714).Conclusion:MSCT dynamic enhancement and post-treatment technology can help the preoperative pathological staging of colon cancer,with high accuracy,and has a high consistency with postoperative pathological diagnosis,with high clinical value.
