摘  要:目的:系统评价基于虚拟现实技术的平衡训练与传统平衡训练相比,对脑卒中偏瘫病人平衡功能的影响。方法:检索the Cochrane Library、EMbase、PubMed、WOS、EBSCO、Elsevier、Springer、中国知网(CNKI)、万方数据库(WanFang Data)、维普数据库(VIP)和中国生物医学文献数据库(CBM),搜索基于虚拟现实技术的平衡训练与传统平衡训练在脑卒中偏瘫病人中应用比较的随机对照试验,检索时限为2015年1月—2020年5月。由2名研究人员独立筛选文献、提取资料并评价文献质量,采用RevMan 5.4软件进行Meta分析。结果:共纳入9篇文献,涉及405例脑卒中偏瘫病人。Meta分析结果显示,基于虚拟现实技术平衡训练与传统平衡训练相比,能提高Berg平衡量表(BBS)得分,缩短起立-行走计时测试(TUGT)时间,平衡测试仪(系统)测定的试验组中心压力参数、极限稳定系数、足底压力分布等结果较对照组改善明显,但对功能性前伸试验(FRT)得分改善差异无统计学意义。结论:基于虚拟现实技术的平衡训练与传统平衡训练相比,可更好地提高脑卒中偏瘫病人平衡能力。受纳入研究质量与数量的限制,本研究的结论需更多高质量研究予以验证。Objective:To systematically evaluate the influence of balance training based on virtual reality technology compared with traditional balance training on the balance function of stroke patients with hemiplegia.Methods:Randomized controlled trials comparing the application of balance training based on virtual reality technology and traditional balance training in stroke patients with hemiplegia were searched from the Cochrane Library,EMbase,PubMed,WOS,EBSCO,Elsevier,Springer,CNKI,WanFang Data,VIP,and CBM,the retrieval time was from January 2015 to May 2020.Two researchers independently screened the literature,extracted the data and evaluated the literature quality.RevMan5.4 software was used for Meta-analysis.Results:A total of 9 articles were included,including 405 stroke patients with hemiplegia.Meta-analysis results showed that compared with traditional balance training,balance training based on virtual reality technology could improve BBS score,shorten TUGT time,balance tester(system)determination of the experimental center of pressure parameters,stability limit coefficient,plantar pressure distribution results was improved obviously,but there was no significant difference in FRT score.Conclusion:Compared with traditional balance training,balance training based on virtual reality technology can better improve the balance ability of stroke patients with hemiplegia.Limited by the quality and quantity of included studies,the conclusions of this study need to be verified by more high-quality studies.
