摘  要:目的:探讨马尾神经损害的神经电生理特征。方法:选择2015年1月至2020年12月在贵州省人民医院住院并确诊为马尾损害的180例患者,采用肌电图/诱发电位仪检查所有患者双侧脊旁肌及肛门括约肌肌电图、胫神经和腓总神经传导速度、潜伏期及波幅、腓浅神经和腓肠神经传导速度及潜伏期,并记录腓总神经和胫神经F波的出波率及潜伏期、下肢体感诱发电位(LSEP)及阴部诱发电位。结果:运动神经传导检查异常率为83.3%(300/360条),阴部诱发电位异常率为67.8%(122/180条),脊旁肌、肛门括约肌出现失神经电位异常率为57.4%(620/1080块),F波异常率为33.3%(120/360条),LSEP异常率为27.2%(98/360条),感觉传导正常。各异常率比较差异具有统计学意义(P<0.05)。结论:马尾损害中运动神经传导和阴部诱发电位异常率最高,可作为早期马尾损害的敏感指标。Objective:To investigate the neurophysiological characteristics of cauda equina nerve injuries.Methods:One hundred and eighty patients with cauda equina injuries hospitalized in Guizhou Provincial People's Hospital from January 2015 to December 2020 were enrolled in the study.Electromyogram of bilateral paraspinal muscles and anal sphincter,conduction velocity,latency and amplitude of tibial nerve and common peroneal nerve,conduction velocity and latency of superficial peroneal nerve and sural nerve were recorded,and the occurrence rate and latency of F wave of common peroneal nerve and tibial nerve,lower limb somatosensory evoked potential(LSEP)and pudendal evoked potential were examined.Results:The abnormal rate of motor nerve conduction was 83.3%(300/360),the abnormal rate of pudendal evoked potential was 67.8%(122/180),the abnormal rate of denervation potential of paraspinal muscle and anal sphincter was 57.4%(620/1080),the abnormal rate of F wave was 33.3%(120/360),the abnormal rate of LSEP was 27.2%(98/360),and sensory conduction was normal.There were statistically significant differences among the abnormal rates(P<0.05).Conclusion:The abnormal rate of motor nerve conduction and pudendal evoked potential is the highest in cauda equina injuries,which can be used as a sensitive index of early cauda equina injuries.
