基于中医传承辅助平台探讨温阳法的证治规律

摘　　要：目的:基于中医传承计算平台探讨温阳法的辨证要点及用药规律。方法:以“温阳法”“医案”“中医”“中药”为主题词,检索中国知网(CNKI)、万方数据库、维普数据库(VIP)及古今医案云平台V 2.3.5中有关温阳法的医案;各数据库检索时间为自建库以来至2020年6月30日;采用模糊检索方式,以保证查全率,同时进行二次引用参考文献查询;运用中医传承辅助平台(V 3.0)分析温阳法的辨证要点及用药规律,总结核心处方,挖掘新处方。结果:共纳入相关文献400篇,收录医案635条,涉及方剂631首、中药672味、症状1108个;“温阳法”治疗的高频疾病为水肿、不寐、心悸、泄泻、胸痹;高频证型为脾肾阳虚;高频治法为益气温阳、温阳散寒;主要症状有怕冷、乏力、肢冷、纳差、气短等;高频药物为茯苓、桂枝、白术、黄芪、炙甘草、制附子、当归、党参、干姜、甘草等,药物主治以补虚为主。结论:应用“温阳法”的高频证型、高频症状和核心药物、核心处方均体现了历代“温阳法”治疗相关病症的组方原则以“益气温阳”为主。Objective:To explore the key points of syndrome differentiation and medication rules of Yangwarming method based on TCMISS.Methods:By taking"Yang-warming method""medical cases""TCM""materia medica"as the subject words,the medical cases on Yang-warming method were searched from CNKI,WanFang,VIP and ancient and modern medical records cloud platform V2.3.5;the retrieval time was from the establishment of the database to June 30,2020 for different databases;"fuzzy"retrieval method was used to ensure the recall rate,at the same time,to perform secondary citation reference query;TCMISS(V3.0)was used to analyze the key points of syndrome differentiation and medication rules of Yang-warming method,to summarize core prescriptions and unearth the new prescription.Results:All 400 relevant papers were included,employing 635 medical cases,involving 631 prescriptions,672 herbs and 1108 symptoms;high-frequency diseases using"Yang-warming method"were enema,insomnia,palpitation,diarrhea and chest arthralgia;high-frequency pattern was yang deficiency of spleen and kidney;high-frequency therapeutic methods were benefiting Qi and warming Yang,warming Yang and dispersing cold;the main symptoms were aversion to cold,fatigue,cold limbs,poor appetite and panting;high-frequency herbs were Fuling[Poria cocos(Schw.)Wol],Guizhi(Cinnamomum cassia Presl),Baizhu(Atractylodes macrocephala Koidz.),Huangqi(Astragali radix),Zhigancao(Glycyrrhizae radix et rhizoma praeparata cum melle),prepared aconite,Danggui(Angelicae sinensis radix),Dangshen(Codonopsis radix),Ganjiang(Zingiberis rhizoma),Gancao(Glycyrrhizae radix et rhizoma),and the main indication was tonification.Conclusion:High-frequency pattern,high-frequency symptoms,core drugs and core prescription of using"Yang-warming method"reflect the prescription principle of"Yang-warming method"in the past dynasties for the treatment of the related disease,mainly focusing on"benefiting Qi and warming Yang".
