老年溃疡性结肠炎病人受体蛋白6、降钙素原和白细胞介素-18表达水平及意义分析

摘　　要：目的 探究老年溃疡性结肠炎(UC)病人核苷酸结合寡聚化结构域样受体蛋白6(NLRP-6)、降钙素原(PCT)和白细胞介素-18(IL-18)表达水平及意义。方法 选取2019年1月至2020年12月河南大学第一附属医院收治的72例老年UC病人,活动期52例,缓解期20例(轻度18例、中度20例、重度14例),免疫组化法检测肠黏膜NLRP-6蛋白表达,荧光定量PCR法检测肠黏膜NLRP-6、IL-18基因表达,酶联免疫吸附法检测血清IL-18,双抗夹心免疫化学发光法检测血清PCT水平,评估临床意义。结果缓解期病人NLRP-6基因和蛋白表达分别为(0.88±0.29)和(0.025±0.006),均高于活动期的(0.58±0.25)和(0.012±0.004),而缓解期病人IL-18基因表达、血清IL-18、血清PCT水平分别为(0.15±0.04)、(101.24±17.16)ng/L和[0.13(0.13,0.17)]μg/L,均低于活动期的(0.40±0.26)、(198.04±110.53)ng/L和(1.11±0.38)μg/L(均P<0.05);活动期重度病人NLRP-6基因和蛋白表达分别为(0.31±0.15)和(0.007±0.002),中度病人分别为(0.56±0.17)、(0.012±0.002),均低于轻度病人的(0.82±0.15)、(0.015±0.013)(均P<0.05);活动期重度病人IL-18基因表达、血清IL-18、血清PCT水平分别为(0.56±0.32)、(370.55±54.39)ng/L和(1.43±0.23)μg/L,中度病人分别为(0.42±0.22)、142.36(133.34,162.12)ng/L和0.13(1.25±0.17)μg/L,均高于轻度病人的(0.26±0.14)、(124.34±14.29)ng/L和(0.71±0.28)μg/L](均P<0.05);重度病人NLRP-6基因和蛋白表达低于中度病人,IL-18基因表达、血清IL-18、血清PCT水平高于中度病人(均P<0.05);活动期病人NLRP-6基因和蛋白表达均与IL-18基因表达、血清IL-18、PCT及疾病活动度负相关,IL-18基因表达与血清IL-18、PCT及疾病活动度正相关,血清IL-18与血清PCT、疾病活动度正相关,血清PCT与疾病活动度正相关;NLRP-6基因和蛋白表达、IL-18基因表达、血清IL-18、血清PCT评估UC活动期中重度的AUC(95%CI)分别为0.92(0.82~0.98)、0.904(0.79~0.97)、0.74(0.60~0.85)、0.86(0.74~0.94)、0.98(0Objective To investigate the expression levels and significance of nucleotide-binding oligomerization domain-like receptor protein 6(NLRP-6),procalcitonin(PCT) and interleukin-18(IL-18) in elderly patients with ulcerative colitis(UC).Methods A total of 72 elderly UC patients admitted to the First Affiliated Hospital of Henan University from January 2019 to December 2020 were selected,including 52 cases(18 mild,20 moderate,and 14 severe) in the active stage and 20 cases in the remission stage.The expression of NLRP-6 protein in intestinal mucosa was detected by immunohistochemistry,and the expression of NLRP-6 and IL-18 genes in intestinal mucosa was detected by fluorescence quantitative PCR.Serum IL-18 was detected by enzyme-linked immunosorbent assay,and serum PCT levels were detected by the double-antibody sandwich immunochemiluminescence method to evaluate the clinical significance.Results The gene and protein expression levels of NLRP-6 in patients in the remission stage were 0.88±0.29 and 0.025±0.006,respectively,which were higher than those in the active stage(0.58 ± 0.25) and(0.012 ± 0.004).The gene levels of IL-18,and serum IL-18 and PCTs in remission stage were 0.15 ± 0.04,101.24 ± 17.16 ng/L and 0.13(0.13,0.17) μg/L,respectively,which were lower than those in the active stage(0.40 ± 0.26),(198.04 ± 110.53) ng/L and(1.11 ± 0.38) μg/L,all of which were significantly different(P <0.05).The expression levels of the NLRP-6 gene and protein of in severe patients at active stage were 0.31±0.15 and 0.007±0.002,respectively,and those in moderate patients were 0.56±0.17 and 0.012±0.002,respectively,which were lower than those in mild patients(0.82±0.15) and(0.015±0.013),respectively.The difference was statistically significant(P < 0.05).The expression of IL-18 gene,serum IL-18 and serum PCT levels in severe patients at active stage were 0.56±0.32,370.55±54.39 ng/L and 1.43±0.23 μg/L,respectively,in moderate patients(0.42 ± 0.22),142.36(133.34,162.12) ng/L and 0.13(1.25 ± 0.17) μg/L,which w
