摘 要:目的探讨超声测量视神经鞘直径(ONSD)与急性颅内高压合并脑疝猪动物模型颅内压(ICP)变化的关系。方法选择普通健康广西巴马小型猪8头,建立急性颅内高压合并脑疝的猪模型。于脑室内置入ICP探测条监测ICP变化,经颅多普勒超声测量实验猪造模前后ONSD,并比较造模前后猪ICP平均值与双眼ONSD,分析ICP与ONSD的相关性。结果建立急性颅内高压合并脑疝模型的实验猪,在造模前后的ICP平均值比较,差异有统计学意义(P<0.001);双眼ONSD在造模前后平均值比较,差异具有统计学意义(P<0.001)。Pearson相关性分析显示,ONSD与ICP呈正相关(r=0.963,P<0.001)。结论床旁超声可快速检测实验猪ONSD变化,对猪急性颅内高压合并脑疝模型ICP变化的评估有重要意义,为超声评估ICP变化的临床推广提供了基础研究依据。Objective To investigate the relationship between the diameter of optic nerve sheath measured by ultrasound and the changes of intracranial pressure in swine model of acute intracranial hypertension complicated with cerebral hernia.Methods Eight healthy Guangxi Bama mini swines were selected to establish the model of acute intracranial hypertension complicated with cerebral hernia,and the changes of intracranial pressure were monitored.The diameter of optic nerve sheath before and after modeling was measured by transcranial Doppler ultrasound,and the relationship between them was discussed.Results The models of acute intracranial hypertension complicated with cerebral hernia were established by all 8 experimental animals.There was significant difference in the average intracranial pressure before and after modeling(P<0.001).There was significant difference in the diameter of optic nerve sheath before and after modeling(P<0.001).Pearson correlation analysis showed that the diameter of optic nerve sheath was positively correlated with intracranial pressure(r=0.963,P<0.001).Conclusion Bedside ultrasound can quickly detect the changes of optic nerve sheath diameter,which is of great significance for the evaluation of intracranial pressure changes in swines with acute intracranial hypertension complicated with cerebral hernia model,and provides a basic research basis for ultrasound to evaluate the changes of intracranial pressure.
