实时PCR检验在高危型人乳头瘤病毒诊断中的价值分析

摘　　要：目的研究高危型人乳头瘤病毒(HR-HPV)应用实时聚合酶链式反应(PCR)检验诊断的临床价值。方法选取73例高危型人乳头瘤病毒患者,分别进行实时PCR检验、第2代杂交式捕获法检验,对比两种检验不同程度宫颈病变检出率、人乳头瘤病毒(HPV)阳性检出率及诊断效能。结果73例患者中不同程度宫颈上皮内瘤变35例,鳞状上皮病变和慢性宫颈炎32例。实时PCR检验鳞状上皮病变和慢性宫颈炎检出率高于第2代杂交式捕获法检验,差异有统计学意义(P<0.05),但两种检验宫颈病变检出率对比差异无统计学意义(P>0.05)。实时PCR检验HPV阳性检出率为75.34%,明显高于第2代杂交式捕获法检验的58.90%,差异有统计学意义(P<0.05)。实时PCR检验诊断准确率、特异性均高于第2代杂交式捕获法检验,差异有统计学意义(P<0.05),两种检验方法敏感性对比差异无统计学意义(P>0.05)。结论高危型人乳头瘤病毒患者应用实时PCR检验诊断价值较高,其特异性和敏感性较高,在临床患者病情诊断中具有重要作用,值得临床推广。Objective To study the clinical value of real-time polymerase chain reaction(PCR)in the diagnosis of high-risk human papillomavirus(HR-HPV).Methods 73 patients with high-risk human papillomavirus were selected for real-time PCR test and second-generation hybrid capture test.The detection rate of cervical lesions,positive detection rate of human papillomavirus(HPV)and diagnostic efficiency of the two tests were compared.Results Among the 73 patients,there were 35 cases of cervical intraepithelial neoplasia,32 cases of squamous epithelial lesions and chronic cervicitis.The detection rates of squamous epithelial lesions and chronic cervicitis by real-time PCR were significantly higher than those by the second generation hybrid capture method(P<0.05),but there was no significant difference between the two tests(P>0.05).The positive detection rate of HPV by real-time PCR was 75.34%,which was significantly higher than 58.90% by the second generation hybrid capture method(P<0.05).The diagnostic accuracy and specificity of real-time PCR test were higher than that of the second generation hybrid capture test,the difference was statistically significant(P<0.05),and there was no statistically significant difference in the sensitivity of the two test methods(P>0.05).Conclusion The application of real-time PCR in the diagnosis of high-risk human papillomavirus patients has high diagnostic value,high specificity and sensitivity.It plays an important role in the diagnosis of clinical patients,and is worthy of clinical promotion.
