摘　　要：目的 探讨自体双层耳屏软骨膜夹层法鼓室成形术治疗鼓膜穿孔患者的效果。方法 选取2018年6月~2019年3月天水市第一人民医院耳鼻喉科收治的80例患者鼓膜穿孔患者作为研究对象,采用随机数字表法分为研究组和对照组,每组40例。研究组采用自体双层耳屏软骨膜夹层法鼓室成形术治疗,对照组采用颞肌筋膜鼓室成形术治疗。比较两组患者的听力指标(气骨导差、气导听阈)、鼓膜愈合情况(鼓膜愈合率、再穿孔率)。结果 研究组术后的气骨导差和气导听阈低于术前和对照组,比较有统计学差异(P<0.05)。研究组术后的鼓膜愈合率高于对照组,再穿孔率低于对照组,比较有统计学差异(P<0.05)。结论 自体双层耳屏软骨膜夹层法鼓室成形术治疗鼓膜穿孔患者的鼓膜愈合率高,且术后发生再穿孔率低,可明显改善患者的听力情况,具有临床推广价值。Objective To explore the effect of tympanoplasty with autologous double tragus perichondrial dissection in the treatment of patients with tympanic membrane perforation.Methods Eighty patients with tympanic membrane perforation admitted to the Department of Otolaryngology,First People’s Hospital of Tianshui City from June 2018 to March 2019 were selected as the research objects,and were divided into a study group and a control group using a random number table method,with 40 cases in each group.The study group was treated with autologous double tragus perichondrial dissection method,and the control group was treated with temporal myofascial tympanoplasty.The surgical indicators (operating time,intraoperative blood loss,number of days in hospital),hearing indicators (air-bone conduction,air conduction hearing threshold),tympanic membrane healing (tympanic membrane healing rate,reperforation rate) were compared between the two groups of patients.Results The postoperative air-bone conduction difference and air-conduction hearing threshold of the study group were lower than those of the preoperative and the control group,which were statistically different (P<0.05).The postoperative tympanic membrane healing rate of the study group was higher than that of the control group,and the reperforation rate was lower than that of the control group,which was statistically different (P<0.05).Conclusion Tympanoplasty with autologous double-layer tragus and perichondrial dissection has a high tympanic membrane healing rate in patients with tympanic membrane perforation,and a low postoperative reperforation rate,which can significantly improve the hearing of patients and has clinical promotion value.
