内镜下金属钛夹联合药物局部注射法治疗溃疡性上消化道出血的效果分析

摘　　要：目的:探讨用内镜下金属钛夹联合药物局部注射法治疗溃疡性上消化道出血(UGH)的效果。方法:选择重庆市东南医院2019年11月至2021年1月期间收治的78例溃疡性UGH患者作为观察对象。按照随机数表法将其分为对照组和观察组,每组各有患者39例。对照组患者接受常规药物治疗,观察组患者在此基础上用内镜下金属钛夹联合药物局部注射法进行治疗。比较两组患者的止血效果、应激反应指标及治疗后发生不良反应的情况。结果:观察组患者止血的总有效率高于对照组患者,差异有统计学意义(P<0.05)。治疗后,观察组患者血清皮质醇(Cor)、生长激素(GH)和抗利尿激素(ADH)的水平均低于对照组患者,差异有统计学意义(P<0.05)。治疗后,两组患者均未发生不良反应。结论:在对溃疡性UGH患者进行常规药物治疗的基础上,用内镜下金属钛夹联合药物局部注射法对其进行治疗能显著提高止血的效果,减轻其机体的应激反应,且治疗的安全性较高。Objective:To investigate the effect of endoscopic titanium clip combined with local drug injection in the treatment of ulcerative upper gastrointestinal bleeding(UGH).Methods:A total of 78 patients with ulcerative UGH who were admitted to Southeast Hospital of Chongqing from November 2019 to January 2021 were selected as subjects.According to the random number table method,they were divided into control group and observation group,with 39 patients in each group.The control group received conventional drug therapy,and the observation group was treated with endoscopic titanium clip combined with local drug injection.The hemostatic effect,stress response index and the occurrence of adverse reactions after treatment were compared between the two groups.Results:The total effective rate of hemostasis in observation group was higher than control group,the difference was statistically significant(P<0.05).After treatment,the serum levels of cortisol(Cor),growth hormone(GH)and antidiuretic hormone(ADH)in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After treatment,there were no adverse reactions in 2 groups.Conclusion:On the basis of conventional drug therapy for patients with ulcerative UGH,endoscopic titanium clip combined with local drug injection can significantly improve the hemostatic effect and reduce the stre ss response of the body,and the safety of treatment is high.
