呼吸道抗原快速三联检与咽拭子培养联合检测在婴幼儿和儿童呼吸道感染中的应用

摘　　要：目的:为探讨呼吸道抗原快速三联检与咽拭子培养联合检测在婴幼儿和儿童呼吸道感染中的应用价值。方法:将2018年12月~2019年12月本院的呼吸道感染患儿230例作为研究组,所有入组对象留取鼻咽拭子标本,进行呼吸道抗原快速三联检与咽拭子培养检测,上述两种检测方法任意一种检测结果为阳性则判定为联合检测阳性,对比分析呼吸道抗原快速三联检以及咽拭子培养单独与联合检测阳性率差异。结果:呼吸道抗原快速三联检检测共检测出阳性病例52例,其中呼吸道核胞病毒阳性14例,腺病毒阳性7例,肺炎支原体31例;咽拭子培养共检测出阳性病例150例,共分离培养出病原菌179株,其中革兰氏阳性菌78株,革兰氏阴性菌107株;联合检测阳性率92.61%高于单独吸道抗原快速三联检阳性率22.61%,高于单独咽拭子培养70.00%(P<0.05);抗原快速三联检联合咽拭子对下呼吸道感染检测阳性率91.16%高于上呼吸道感染81.93%(P<0.05)。结论:呼吸道抗原快速三联检与咽拭子培养联合检测对婴幼儿和儿童呼吸道感染的诊断具有重要价值,可有效提高检测阳性率,且对下呼吸道感染的诊断要较上呼吸道感染患儿敏感。Objective:To explore the application value of rapid triple detection of respiratory antigens combined with throat swab culture in respiratory infection of infants and children.Methods:A total of 230 children with respiratory infection in the hospital were enrolled as study group between December 2018 and December 2019,The nasopharyngeal swab specimens from all objects were collected for rapid triple detection of respiratory antigens combined with throat swab culture.If there was positive by either of the above two detection methods,there was positive in combined detection.The differences in positive rates of rapid triple detection of respiratory antigens,throat swab culture alone and their combination were compared and analyzed.Results:There were 52 positive cases detected by rapid triple detection of respiratory antigens,including 14 cases with positive respiratory syncytial virus,7 cases with positive adenovirus and 31 cases with positive Mycoplasma pneumoniae.There were 150 positive cases(179 strains of pathogens)detected by throat swab culture,including 78 strains of Gram-positive bacteria and 107 strains of Gram-negative bacteria.The positive rate of combined detection was 92.61%,higher than that of rapid triple detection of respiratory antigens and throat swab culture alone(22.61%,70.00%)(P<0.05).The positive rate of lower respiratory infection was higher than that of upper respiratory infection detected by rapid triple detection of respiratory antigens combined with throat swab culture(91.16%vs 81.93%)(P<0.05).Conclusion:The rapid triple detection of respiratory antigens combined with throat swab culture is of great diagnostic va lue for respiratory infection in infants and children,which can effectively increase positive rate of detection and is of higher sensitivity in the diagnosis of lower respiratory infection than upper respiratory infection.
