达芬奇机器人减臂法手术在结直肠癌根治术中的近期临床疗效研究

摘　　要：目的:比较达芬奇机器人常规手术与达芬奇机器人减臂法手术对于结直肠癌的近期疗效差异.方法:回顾性收集2021年1月~2021年12月中南大学湘雅三医院胃肠外科收治的93例接受达芬奇机器人辅助下左半、右半结肠癌或低位直肠癌根治术的患者临床资料,根据术中使用机械臂数量的不同,分为常规组(n=34)、减臂组(n=59).统计两组手术情况、术后病理结果、术后恢复情况、术后并发症情况及手术相关费用等临床数据.结果:两组手术均顺利完成,无中转开腹、无严重术中并发症发生及术中死亡病例.两组间在手术时间、术中出血量、淋巴检出数、术后T分期、N分期、术后拔除尿管时间、首次排气时间、首次进食流质时间、术后住院时间、术后并发症等方面,差异均无统计学意义.两组间手术费用比较,减臂组费用明显更低.结论:达芬奇机器人减臂法手术与达芬奇机器人常规手术相比,对于结肠癌及低位直肠癌的近期疗效无明显差异,而在手术费用上,减臂法手术明显更低.Objective To compare the short-term curative effect of da Vinci robotic conventional surgery and da Vinci robotic arm reduction surgery for colorectal cancer.Methods The clinical data of 93 patients who received da Vinci robot-assisted radical resection for left-sided,right-sided colon cancer or low rectal cancer from January 2021 to December 2021 in the Department of Gastrointestinal Surgery,The Tliird Xiangya Hospital of Central South University were retrospectively collected.According to the number of mechanical arms used during surgery,they were divided into conventional group(n=34)and reduced arm group(n=59).The clinical data such as operation,postoperative pathological results,postoperative recovery,postoperative complications,and operation-related expenses were recorded in the two groups.Results The operations in both groups were successfully completed without conversion to laparotomy,serious intraoperative complications and intraoperative deaths.There were no significant differences between the two groups in terms of operation time,intraoperative blood loss,number of lymph nodes detected,postoperative T stage,N stage,postoperative catheter removal time,first exhaust lime,first fluid intake time,postoperative hospital stay and postoperative complications.There was a statistically significant difference in the cost of surgery between the two groups,and the cost of the arm reduction group was significantly lower.Conclusion Compared with conventional robotic surgery,there is no significant difference in the short-term curative effect of da Vinci robotic arm reduction surgery for colon cancer and low rectal cancer,and the surgery cost is significantly lower with arm reduction surgery.
