摘  要:目的:研究阶梯减压技术对于兔急性脑损伤后脑组织的保护作用及其对脑缺血再灌注损伤(CIRI)的影响.方法:建立兔急性加速性脑损伤模型,实验兔随机分为假手术组、阶梯减压组、快速减压组,观察术后24 h后兔脑组织中海人藻酸受体亚基5(GluR5)和海人藻酸受体亚基6(GluR6)的表达水平、脑组织微观病理变化.结果:术后24h,阶梯减压组GluR5的表达明显高于快速减压组,GluR6的表达明显低于快速减压组;脑组织HE染色阶梯减压组明显轻于快速减压组;阶梯减压组术中急性脑膨出的发生率明显低于快速减压组.结论:阶梯减压技术治疗兔急性脑损伤能有效减轻CIRI,降低术中急性脑膨出发生率,实现明显的脑保护作用.Objective To investigate the protective effects of step decompression on cerebral tissue and cerebral ischemia-reperfusion injury(CIRI)in rabbits after acute brain injury.Methods A rabbit model of acute accelerated brain injury was established,and the experimental rabbits were randomly divided into sham operation group,step decompression group and rapid decompression group.The expressions of the glutamate receptors(GluR5 and GluR6)in the brain tissue and the pathological changes of brain tissue were observed 24 hours after operation.Results GluR5 expression in step decompression group was significantly higher than that in rapid decompression group,GluR6 expression was significantly lower than that in rapid decompression group;HE staining of brain tissue in step decompression group was significantly lighter than that in rapid decompression group.The incidence of acute encephalocele in step decompression group was significantly lower than that in rapid decom-pression group.Conclusion Step decompression can effectively reduce CIRI,reduce the incidence of acute encephalocele,and achieve obvious brain protection in rabbits with acute brain injury.
