摘 要:目的探讨前列腺影像报告和数据系统2.1版(PI-RADS v2.1)联合血清前列腺特异性抗原密度(PSAD)对移行带前列腺癌(PCa)的诊断应用价值。方法回顾性分析255例经病理证实的移行带PCa或前列腺增生(BPH)患者的相关资料。由2名经验丰富的MRI诊断医师通过盲法对所有患者MRI图像进行PI-RADS v2.1评分,统计并计算血清PSAD。采用t检验比较各项指标在2组间的差异性,并使用受试者工作特征(ROC)曲线分析PI-RADS v2.1评分、PSAD、PI-RADS v2.1评分联合PSAD对移行带PCa的诊断效能。结果PI-RADS v2.1评分与PSAD在PCa与BPH组间的差异有统计学意义(P<0.05)。根据ROC曲线分析,PI-RADS v2.1评分、PSAD、PI-RADS v2.1评分联合PSAD诊断移行带PCa的曲线下面积(AUC)分别为0.842、0.794及0.869,且PI-RADS v2.1评分联合PSAD的AUC显著高于单独应用PI-RADS v2.1(Z=2.208,P<0.05)。结论PI-RADS v2.1评分与PSAD诊断移行带PCa均有较高的敏感度与特异度,PI-RADS v2.1评分与PSAD联合能进一步提高诊断效能。Objective To explore the diagnostic value of prostate imaging reporting and data system version 2.1(PI-RADS v2.1)combined with serum prostate specific antigen density(PSAD)in the diagnosis of transitional zone prostate cancer(PCa).Methods The data of 255 patients of pathologically confirmed transitional zone PCa or benign prostate hyperplasia(BPH)were analyzed retro-spectively.Two experienced MRI diagnostic physicians blindly scored PI-RADS v2.1 on MRI images of all patients,and calculated the serum PSAD.The t test was used to compare the differences of various indicators between the two groups,and the receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of various indicators for transitional zone PCa.Results The difference between PI-RADS v2.1 score and PSAD in the PCa and BPH groups was statistically significant(P<0.05).According to ROC curve analysis,the area under the curve(AUC)of PI-RADS v2.1 score,PSAD,PI-RADS v2.1 score combined with PSAD for diagnosis of transitional zone PCa were 0.842,0.794 and 0.869,respectively.The AUC of PI-RADS v2.1 score combined with PSAD was significantly higher than that of PI-RADS v2.1 alone(Z=2.208,P<0.05).Conclusion PI-RADS v2.1 score and PSAD have high sensitivity and specificity in the diagnosis of transitional zone PCa,the combination of PI-RADS v2.1 score and PSAD can further improve the diagnostic efficiency.
