增强CT动脉期时相对胃黏膜线显示效果的研究

摘　　要：目的探讨胃癌腹部增强CT扫描对胃黏膜线显示更佳的动脉期时相。方法回顾性选取行全腹部增强CT检查胃癌资料100例,其中动脉期采用30 s及40 s扫描的资料各50例,主要为患者年龄、性别和动脉期图像,测量3种CT值参数,包括胃黏膜线动脉期CT值的最大值(Value AP Max)、最小值(Value AP Min)和平均值(Value AP Mean),并对胃黏膜线显示情况进行主观评分。对年龄分组及性别、评分行卡方检验,CT值参数采用正态分布检验,依据结果采用独立样本t检验,应用受试者工作特征(ROC)曲线分析其诊断效能。最后对CT值进行组内相关系数(ICC)分析。结果年龄分组及性别均无显著差异(χ^(2)=0.00、2.49,P=1.000、0.115)。胃黏膜线显示情况评分有显著差异(χ^(2)=46.29,P<0.001)。参数Value AP Max、Value AP Min、Value AP Mean在30 s和40 s组间存在显著差异(t=-7.727、-8.218和-8.177,P均<0.001),其Value AP Mean分别为87.54 HU和126.78 HU、70.56 HU和105.66 HU及80.05 HU和117.45 HU,曲线下面积(AUC)分别为0.877、0.878和0.883。ICC值分别为0.91、0.88和0.93,观察者间CT值测量一致性优秀。结论胃癌增强CT检查动脉期采用40 s扫描有助于胃黏膜线的显示,可以帮助提高胃癌的检出率。Objective To explore the abdominal contrast-enhanced CT of gastric cancer to find a better arterial phase for showing the gastric mucosal line.Methods Clinical and image data of 100 patients(including age,gender,and arterial phase images;50 cases with 30 s and 50 cases with 40 s in arterial phase)with gastric cancer were retrospectively collected.Three arterial phase CT value parameters were measured,including the maximum,minimum,and mean CT values(Value AP Max,Value AP Min,and Value AP Mean)of gastric mucosal line.The display of the gastric mucosal line was also been scored subjectively.Theχ^(2)test was used to analyze the age group,gender,and scores.The normality distribution of CT value parameters were evaluated by the Shapiro-Wilk test.According to the normality test results,the independent sample t tests were used.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic performance.Finally,the intraclass correlation coefficient(ICC)was used to analyze the CT values.Results There were no significant differences in age group and gender between different arterial phases(χ^(2)=0.00 and 2.49,P=1.000 and 0.115,respectively).There were significant differences in the displaying gastric mucosal lines(χ^(2)=46.29,P<0.001).There were significant differences in the parameters of Value AP Max,Value AP Min,and Value AP Mean between 30 s and 40 s groups(t=-7.727,-8.218 and-8.177,all P<0.001),and their average values were 87.54 HU and 126.78 HU,70.56 HU and 105.66 HU,80.05 HU and 117.45 HU,respectively.The area under the curve(AUC)were 0.877,0.878,and 0.883,respectively.The values of ICC were 0.91,0.88,and 0.93,respectively.The consistency of CT values measured between observers was excellent.Conclusion Using 40 s in arterial phase in the abdominal contrast-enhanced CT scan of gastric cancer helpvS to show gastric mucosal lines,which help increase the detection rate of gastric cancer.
