摘　　要：目的:探讨垂体瘤患者手术中采用经鼻蝶窦手术方式对视力及嗅觉功能改善情况。方法:选择我院2017年12月~2020年12月垂体瘤患者80例作为研究对象,依据手术方法分为观察组40例与对照组40例。两组患者均采取经鼻蝶窦入路肿瘤切除术,观察组以神经内镜为辅助工具,对照组以显微镜为辅助工具。将两组手术前后指标变化进行对比,术后并发症发生率,肿瘤完全清除比例和视力视野情况;术前、术后1周和术后3个月嗅觉功能及视力改善情况。结果:与对照组相比较,观察组患者的手术所用时长和住院日明显短,手术过程中出血量较少术后并发症的发生率小,肿瘤全切率和视力视野改善率高于对照组。两组术后1周经T&T标准嗅觉测试法检测所得评分高于术前;观察组术后3个月T&T标准嗅觉测试评分较术前无明显变化;对照组术后3个月采用T&T标准嗅觉测试法检测所得评分高于术前;观察组术后1周和术后3个月T&T标准嗅觉测试法检测评分低于对照组。结论:神经内镜下经鼻蝶窦入路垂体瘤切除术在垂体瘤治疗中可取得良好效果,可改善患者嗅觉功能和视力水平。Objective To explore the improvement of olfactory function and vision of pituitary tumor resection via nasal sphenoid sinus approach.Methods From December 2017 to December 2020 in our hospital,80 patients with pituitary tumors were selected as the research objects.According to the surgical methods,they were divided into 40 cases in the observation group and 40 cases in the control group.The patients that of in the observation group were treated with neuroendoscopic transsphenoidal pituitary tumor resection,the control group were treated with transsphenoidal pituitary tumor resection with microscope.Comparison of perioperative index changes,postoperative complications,complete tumor clearance rate,improvement of vision and improvement of olfactory function between the two groups before operation,1 week after operation,and 3 months after operation.Results Compared with the control group,the operation time and hospital stay of the observation group were shorter,and the intraoperative blood loss was less.Postoperative complications,in the observation group,were less than control group.The complete tumor clearance rate and vision improvement rate of the observation group were higher control group.The T&T standard olfactory test scores of the two groups at 1 week after operation were higher before the operation;the observation group had no significant changes in the T&T standard olfactory test scores 3 months after the operation compared with before surgery;the T&T standard olfactory test score of the control group at 3 months after operation was higher than that before operation;the observation group’s T&T standard olfactory test scores were lower than control group at 1 week and 3 months after surgery.Conclusion Neuroendoscopic transsphenoidal pituitary tumor resection is effective in treating patients with pituitary tumors,and can improve the patient’s olfactory function and vision.
