摘 要:目的比较3种检测方法对于CD38单抗治疗多发性骨髓瘤后输血前相容性检测的结果,探讨解决CD38单抗干扰输血相容性检测的方法。方法采用DTT处理试剂细胞、献血员细胞,比较抗人球蛋白法、微柱凝胶法.凝聚胺介质法对于4例使用CD38单抗治疗患者的输血前相容性检测项目的检测结果。结果DTT处理细胞后,采用3种方法检测4例使用CD38单抗治疗患者不规则抗体筛查,不规则抗体鉴定.交叉配血实验,结果均为阴性;不使用DTT处理细胞,采用3种方法检测4例患者不规则抗体筛查,不规则抗体鉴定、交叉配血实验,凝聚胺介质法均为阴性,另2种方法均为阳性,其中微柱凝胶法凝集强度更强。结论在输血前相容性检测中凝聚胺介质法不受CD38单抗的影响更加快速,适用于急诊输血。抗人球蛋白法.微柱凝胶法需DTT处理去除CD38单抗的影响.但是微柱凝胶法灵敏度更高.适用于平诊输血。Objective The results of three detection methods were compared for the detection of the compatibility of CD38 monoclonal antibody for multiple myeloma after transfusion,and the methods to solve the problem of CD38 monoclonal anti-interference transfusion compatibility detection were discussed.Methods DTT treatment reagent cells and donor cells were used to compare the detection results of antihuman globulin method,microcolumn gel method and condensed amine medium method for 4 patients treated with CD38 before blood transfusion.Results After DTT treatment,irregular antibody screening,irregular antibody identification and cross match test were detected in 4 patients treated with CD38 monoclonal antibody by three methods,and the results were all negative.The cells were treated without DTT,and 4 patients were tested by three methods;screening for irregular antibodies,identification of irregular antibodies,and cross-matching test.The condensed amine medium method was negative,the other two methods were positive,and microeolumn gel method has stronger agglutination strength.Conclusion In the detection of before transfusion compatibility,the condensed amine medium method is not affected by CD38 monoclonal antibody and is rapider and more suitable for emergency transfusion.The antihuman globulin method and microeolumn gel method require DTT treatment to remove the influence of CD38 monoclonal antibody,but microeolumn gel method has higher sensitivity and is suitable for clinical transfusion.
