Er/Nd:YAG激光与手术治疗小儿上唇系带过短的效果分析

摘　　要：目的分析上唇系带过短患儿开展Er/Nd:YAG激光与上唇系带手术治疗的临床效果。方法选取2019年4月至2020年6月铜川市人民医院收治的上唇系带过短患儿84例,根据就诊时间将其分为两组,各42例;对照组实施传统手术唇系带修整术治疗,研究组采取Er:YAG激光联合Nd:YAG激光治疗,比较两组患儿手术效果、术后疼痛评分、愈合时间等。结果研究组患儿手术时间(7.24±0.75)min、术中出血量(0.41±0.15)mL、愈合时间(7.68±0.90)d,均显著低于对照组的手术时间(11.56±1.13)min、术中出血量(1.67±0.56)mL和愈合时间(10.35±0.94)d,差异有统计学意义(P<0.05)。研究组患儿李克特(Likert)五点,即唇部运动评分、语言功能评分、咀嚼功能评分、出血程度评分、肿胀程度评分,在术后3 h、3 d、1周等时间点均显著低于对照组,差异有统计学意义(P<0.05)。两组患儿视觉模拟疼痛量表(visualo analogure pain scale,VAS)评分,研究组术后3 h、3 d、1周评分均低于对照组,差异有统计学意义(P<0.05)。结论Er/Nd:YAG激光治疗上唇系带过短患儿较传统上唇系带修整术,有助于降低手术出血量,缩短手术时间与愈合时间,降低手术对于患儿负面影响,减轻手术疼痛程度,值得推广。Objective To compare and analyze the effect of upper lip frenulum surgery and Er/Nd:YAG laser treatment in children with short upper lip frenulum.Methods A total of 84 children with short upper lip frenulum admitted to Tongchuan People's Hospital from April 2019 to June 2020 were selected and divided into two groups according to the visiting time,with 42 cases in each group.In the control group,there were 23 boys and 19 girlss,aged 4~12 years,with an average age of(7.24±2.33)years;and the children received traditional surgical frenulectomy.In the research group,there were 26 boys and 16 girls,aged 4~11 years,with an average age(7.50±2.05)years old,performed Er:YAG laser combined with Nd:YAG laser treatment.The surgical effect,postoperative pain score,healing time,etc.were compared between the two groups.Results The operation time(7.24±0.75)min,intraoperative blood loss(0.41±0.15)mL,and healing time(7.68±0.90)d in the study group were significantly lower than those in the control group(11.56±1.13)min,(1.67±0.56)mL,(10.35±0.94)d,respectively.The difference was statistically significant(P<0.05).The five Likert points in the study group,namely the lip movement score,language function score,masticatory function score,bleeding degree score,swelling degree score,were significantly lower than those in the control group at 3 h,3 d,and 1 week after operation.The difference was statistically significant(P<0.05).The VAS pain scores of the two groups were compared,and the scores of the study group at 3 h,3 d,and 1 week after the operation were all lower than those of the control group,and the difference was significant(P<0.05).Conclusion For the treatment of children with short upper lip frenulum,compared with the upper lip frenulum trimming,Er/Nd:YAG laser,which can help reduce the amount of surgical blood loss,shorten the operation time and healing time,and reduce the operation cost to patients and negative effects on children,reduce the degree of pain in surgery.It is worth promoting.
