摘  要:原发性恶性骨肿瘤在儿童及青少年中有较高的发病率和死亡率。虽然化疗药物联合手术治疗使恶性骨肿瘤患者的5年生存率逐渐提高,但随着治疗进展促进耐药产生及复发的问题使化疗药的应用受到极大限制。恶性骨肿瘤微环境的研究揭示,肿瘤免疫环境的改变与肿瘤的发生、发展及耐药有着密切的联系。这些治疗方法的局限性促进了对肿瘤免疫疗法如嵌合抗原受体T细胞(chimeric antigen receptor T cell,CAR-T)疗法、免疫检查点阻断疗法、自然杀伤(natural killer,NK)细胞输注疗法、溶瘤病毒疗法等在恶性骨肿瘤中的基础研究和临床应用探索。本文拟对近年来在恶性骨肿瘤中开展的上述免疫疗法加以概述,并对未来发展趋势作一展望。Primary malignant bone tumor has a high morbidity and mortality rate in children and adolescents.Although the 5-year survival rate of patients with malignant bone tumor improved by the chemotherapeutics combined with surgery,the emergence of drug resistance and immune escape during the treatment greatly limited the application of chemotherapy drugs.Meanwhile,investigation on the microenvironment of malignant bone tumors revealed that the changes of tumor immune microenvironment are closely associated with the occurrence,development and drug-resistance of tumors,which contributed to the basic research and clinical application of immunotherapies in primary malignant bone tumor,such as CAR-T,immune checkpoint blocking,NK cell infusion,oncolytic virus therapies.Here,the above immunotherapy in primary malignant bone tumors is summarized and the development trend of malignant bone tumor is prospected.
