摘 要:目前,急性髓细胞白血病(AML)治疗仍然主要基于标准化疗和异基因造血干细胞移植(allo-HSCT)。虽然治疗后AML患者无病生存(DFS)和总体生存(OS)均有所改善,但是AML老年患者的疗效改善非常有限。部分患者经标准方案治疗后,无法获得完全缓解(CR),或者在获得CR后复发。这类复发/难治性患者成为目前AML治疗相关研究的重点。丙戊酸通过诱导AML细胞分化、促进凋亡、逆转细胞耐药等机制,发挥抗AML作用。笔者拟就丙戊酸抗AML机制的最新研究进展进行综述,旨在为丙戊酸应用于复发/难治性AML患者的治疗提供理论依据。Currently,treatment of acute myeloid leukemia(AML)is still mainly based on the use of standard chemotherapy and allogeneic hematopoietic stem cell transplantation(allo-HSCT).Although disease-free survival(DFS)and overall survival(OS)have improved in patients with AML after treatment,treatment improvement of elderly patients is very limited.Some patients cannot achieve complete remission(CR)after standard treatment,or relapse after CR.Such relapsed/refractory patients have become the focus of current AML treatment research.Valproic acid plays anti-AML effect by inducing differentiation,promoting apoptosis,and reversing drug resistance of AML cells.This article intends to review the anti-AML mechanisms of valproic acid to provide theoretical basis for valproic acid in treatment of patients with relapsed/refractory AML.
