摘 要:目的通过对大连地区新生儿17α羟孕酮(17-OHP)筛查结果的分析,了解本地区足月儿、早产儿、正常体重儿及低体重儿的17-OHP浓度分布情况并制定合适的切值。方法采集45504例新生儿足跟血,检测足跟血17-OHP浓度,并采用百分位数法确定切值。结果足月儿和早产儿的17-OHP浓度比较,P<0.01。正常体重儿和低体重儿的17-OHP浓度比较,P<0.01。结论孕周与新生儿体重显著影响新生儿17-OHP浓度水平,建议足月和正常体重新生儿17-OHP切值设为11 nmol/L,而早产儿、低体重儿17-OHP切值设为30 nmol/L,以减少假阳性率和召回复查率。Objective To study the concentration distribution of 17-OHP in term infants,premature infants,normal weight infants and low birth weight infants to determine the appropriate cut-off value through analyzing the screening results of 17-OHP in newborns in Dalian.Methods Heel blood samples of 45504 newborns were collected to detect the concentration of 17-OHP,and the cut-off value was determined by percentile method.Results There were significant differences between the term infants and premature infants(P<0.01).there were significant differences between innormal birth weight infants and low birth weight infants(P<0.01).Conclusion The results show that gestational week and birth weight have influences on the concentration of 17-OHP in neonates.It is suggested that the 17-OHP concentration cut-off value should be set to 11nmol/L for term infants and normal weight infants,and 30nmol/L for premature infants and low birth weight infants.
